<p>Position Summary:</p> <p>The Health Home Director is responsible for overseeing the Health Home Program. This position ensures quality services are provided and that services meet program objectives. Provides input into the strategic planning process and ensures program services meet organizational guidelines, funder requirements and accreditation standards.</p> <p>This position fulfills the Health Home Director role per DMH Health Home guidelines.</p> <p>Essential Functions:</p> <ul> <li>Oversees program services and ensures that services are meeting organizational guidelines, funder requirements and accreditation standards. Reviews compliance requirements, ensures procedures are in place to meet requirements and develops action plans to ensure compliance. Provides input and feedback into creating and revising policies and procedures and oversees implementation. </li><li>Works to continually improve quality of program services delivered. Develops action plans and ensures that deficiencies are corrected. Ensures high quality Health Home services through direct supervision of Health Home Associate Directors and Care Coordinators and indirect supervision of clinical team. </li><li>Establishes effective working relationships with internal Health Home team as well as external stakeholders such as DMH, service coordinators and the public, including marketing Health Home services throughout the state. Leads or collaborates within internal team meetings, DMH collaboratives and other committees and ad hoc projects as assigned. </li><li>Manages the preparation and maintenance of program reports, with a focus on population health. Reviews and analyzes reports related program operations; identifies areas for improvement; develops and implements action plans; and ensures project completion within established timeframes. </li><li>Oversees employee performance and provides effective coaching, feedback and performance evaluations. Ensures team members understand their role and are held accountable for their objectives. Promotes employee development in relation to providing quality services and developing employee skills. Ensures employees are in compliance with training and employee documentation requirements. </li><li>Responsible for creating annual budget with other directors and VPs. Responsible for monitoring and making adjustments with monthly financials. </li><li>Participates in the development and implementation of strategic and programmatic outcomes and initiatives. </li></ul> <p>Position Competencies:</p> <ul> <li>Knowledge of disabilities, medical care and behavioral supports. Participates in organizational and funder trainings, and external training opportunities for ongoing growth and development. </li><li>Able to interpret and analyze health data and set programmatic health goals. </li><li>Able to multitask and prioritize activities to ensure timelines are met. </li><li>Establishes and maintains successful interpersonal relationships and works cooperatively in group situations. Able to develop professional relationships with a variety of internal and external stakeholders. </li><li>Demonstrates effective leadership and management skills by clearly delegating responsibilities, providing regular performance feedback, building productive teams, developing employee skills and encouraging employee growth and development. </li><li>Intermediate-level proficiency with Microsoft Office products, including Excel, and Electronic Health Record systems. </li></ul> <p>Qualifications:</p> <ul> <li>Must have bachelor's degree in human services field (social work, psychology, counseling, etc.). </li><li>Must have at least 3 years of management experience in IDD, behavioral health or medical systems. </li><li>Must have experience using Microsoft Office suite and must have intermediate-level skill with Excel. </li><li>Previous experience working with Electronic Health Record systems or other data systems preferred. </li></ul> <p>Additional Requirements:</p> <ul> <li>This position typically works weekday, daytime hours. May work evenings and weekends as need. </li><li>May travel to other locations for meetings. May have overnight travel. </li><li>Must have current driver's license and proof of insurance. </li><li>Must complete and maintain all required trainings/certifications. </li><li>Must maintain confidential information in accordance with HIPAA regulations. </li></ul>
