We are seeking a RN Non-Institutional ACT Team who is motivated to help individuals and families navigate life's challenges and achieve long-term wellness. If you want to use your skills to build a healthier, more resilient community, we encourage you to apply.</p> <p>This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Waccamaw Center for Mental Health, all counties - Horry, Williamsburg and Georgetown.</p> <p>As the Registered Nurse, Non-Institutional, you will serve on the Assertive Community Treatment (ACT) team providing evidence-based services in an intensive non-residential treatment and rehabilitative service in accordance with the fidelity model. This is a single fixed point of responsibility for treatment, rehabilitation, and support needs for patients with Serious Mental Illness (SMI) who require higher levels of community care and have not bell support in lower level of care options.</p> <p>Responsibilities for the Registered Nurse Non-Institutional:</p> <ul> <li>Provide direct service to ACT beneficiaries to include but not limited to assessing the health, social, psychiatric and medical needs of WCMH patients. </li><li>Administer scheduled intramuscular medications to patients as ordered by prescribers. </li><li>Monitor patients on regular basis for beneficial and side effects and other medication related issues </li><li>Develop strategies to maximize medications as prescribed. </li><li>Engage in health promotion, prevention and educations activities. </li><li>Ensure patient is seen in accordance with their needs and medical and psychiatric appointments. </li><li>Complete clinical documentation using collaborative documentation and ensure services are documented in accordance with SCDBHDD/QA/CARF requirements. </li><li>Participate in clinical supervision and daily treatment meetings. </li><li>Timely completion of prior authorizations is required. </li><li>Pursue professional development through collegial consultation, continuing education and licensure. </li><li>Promote and help maintain the health of individuals and families through teaching, counseling and rehabilitative measures. </li><li>Maintain documentation of charts and other records in accordance with State, Federal, Local, OMH, WCMH, Quality Assurance, Corporate Compliance and CARF Standards. </li><li>Knowledge of medical and nursing terminology. </li><li>Ability to evaluate a client's medical status and needs and use knowledge and judgement to respond appropriately. </li><li>Plan, implement and evaluate client medical care plans. </li><li>Provide individual health education and counseling to patient. </li><li>Knowledge or privacy laws. </li></ul> <p>Minimum and Additional Requirements</p> <ul> <li>Graduation from an accredited nursing school. </li><li>Licensed as a registered nurse by the South Carolina Board of Nursing through LLR. </li><li>One year experience working with adults diagnosed with serious mental illness and knowledge of psychiatric medications. </li><li>Prior work experience in a mental community program. </li></ul> <p>Additional Requirements:</p> <ul> <li>Ability to lift up to fifty (50) pounds; ability to see, hear at normal volumes, stand, walk, reach, pull, stoop, bend and climb at least two (2) flights of stairs. </li><li>Requires flexible schedule that include services delivered in the community and patient's home. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Bilingual abilities in English and Spanish (or another language) are a plus. </li></ul> <p>Additional Comments</p> <p>Post hire, Employee Must:</p> <ul> <li>Be able to provide a 10-year DMV driving records. </li><li>Be able to use a state vehicle for work related purposes. </li><li>Use clinical judgement while working independently and be self-directed. </li><li>Communicate effectively. </li><li>Ability to function as an ACT team member. </li><li>Have capacity to establish and maintain effective working relationships with patients, families and representatives of other professional disciplines. </li><li>Be able to work with repetitive patient's behaviors. </li><li>Be able to show evidence of clinical experience providing direct patient care. </li><li>Meet OMH credentialing requirements. </li></ul> <p>The Department of Behavioral Health and Developmental Disabilities is committed to providing equal employment opportunities to all applicants and does not discriminate on the basis of race, color, religion, sex (including pregnancy, childbirth, or related medical conditions including, but not limited, to lactation), national origin, age (40 or older), disability or genetic information.</p> <p>Supplemental questions are considered part of your official application.
