<p>Position Summary</p> <p>The LCSW provides quality clinical consultation addressing patient psycho-social concerns in collaboration with the integrated primary care team.</p> <p>Essential Duties & Functions</p> <ul> <li>Provide cognitive and behavioral interventions to individuals (adult and pediatric patients) with a behavioral health problem </li><li>Provide classes in a format appropriate for a primary care setting (i.e., stress management, sleep enhancement and chronic pain) </li><li>Consult with primary care physicians (PCP) regarding patient care, as requested </li><li>Timely follow-up on all PCP referrals via same-day verbal and/or written communication; timely documentation in the patients' medical record </li><li>Deliver brief presentations/trainings to primary care team members regarding behavioral health related issues and BHC services provided </li><li>Participate in the development, implementation, evaluation, and revision of clinical pathway programs </li><li>Consult with legal authorities, as directed </li><li>Support the Mission, Vision and Values of TFHC </li><li>Effectively manage a full daily patient schedule as defined by the BHC manual </li><li>May include supervision of peer providers who are in training or not yet fully licensed </li><li>Attend and participate in meetings, committees, and conferences, as directed </li><li>Other duties, as assigned. </li></ul> <p>Required Education, Certifications, Licenses, & Training</p> <ul> <li>Master's Degree in Social Work or related field with a minimum of (2) years' experience as a therapist OR a Doctoral Degree in Psychology, with a minimum of (2) years' experience as a therapist </li><li>Current BLS Certification </li><li>Current and unencumbered license to practice in the State of Florida </li><li>Licensed Social Worker </li></ul> <p>Required Years of Experience</p> <ul> <li>(2) years' experience providing mental health services in a community-based setting preferred </li></ul> <p>Required Knowledge, Skills, and/or Abilities</p> <ul> <li>Must be bilingual (Spanish) </li><li>Ability to navigate and enter data into an electronic health record </li><li>Ability to work independently, adhering to all TFHC, HIPAA, accreditation, Federal and State rules and regulations </li><li>Ability to work with persons from a wide diversity of social, ethnic, and economic backgrounds </li><li>Strong communication skills, both written and oral </li><li>Skillful and tactful in working with patients who may be physically and/or mentally ill, uncooperative, fearful, and emotionally distraught </li><li>Strong organizational and problem-solving skills; Ability to work in a fast-paced environment </li><li>Excellent interpersonal skills: ability to work collaboratively with primary care team members, patients, and other Behavioral Health professionals </li></ul>
