This</p> <p>includes communication with patients/families, staff and providers, as well as rotating resident.</p> <p>EEO/AA/Disability/Veteran</p> <p>Responsibilities</p> <p>Main duties:</p> <p>1.1 Coordinates initial patient contact by attending Tumor Board Conference and compiles necessary list of</p> <p>presented patients, identifying additions or deletions in collaboration with attending physicians and</p> <p>house staff. Takes meeting minutes in compliance with YNHH Cancer Committee.</p> <p>1.2 Maintains pre-admission contact with patients' families and referring physicians. Conveys pertinent</p> <p>medical information in collaboration with the attending physicians and/ or houses staff to patients'</p> <p>primary and referring physicians and insurance providers.</p> <p>2.1 During initial and pre-admission contacts with patients' families, consults with the service financial</p> <p>analyst to determine how best to address issues which may arise regarding patient benefits and</p> <p>medical reimbursements.</p> <p>3.1 Facilitates diagnostic, post-operative and/or treatment follow-up by participating regularly in regular</p> <p>rounds with the provider team.</p> <p>4.1 Collaborates with the patient's primary and associate nurses, as well as other members of the health</p> <p>care team regarding current medical plan of care. Attends interdisciplinary meetings, discharge</p> <p>planning rounds and family meetings as a representative of the service. Functions as a nurse resource</p> <p>to the staff for patients on the service and consultation for patients seen by the service .</p> <p>5.1 Maintains postoperative follow-up of One Day Surgery patients through telephone communication.</p> <p>Assures continuity of care.</p> <p>6.1 Maintains inpatient follow-up, operations, assesses patient needs, concerns and questions, and</p> <p>provides education and information. Collaborates with the appropriate individuals to facilitate the</p> <p>transition and coordination of follow-up after clinic visits, and/or follow-up by consulting services.</p> <p>Communicates with referring and primary physicians following patient discharge or</p> <p>7.1 Transfer by providing updated medical information, which includes a report of the surgical procedure(s)</p> <p>and applicable details and continuing care plan arranged.</p> <p>8.1 Participates in enhancing the professional development of the nursing staff and other health care</p> <p>providers involved in the care of the oncology patients. Facilitates self care of patients through patient</p> <p>and family education.</p> <p>9.1 Functions as the educational resource to staff nurses and other health care providers in the disease unit</p> <p>and/or quality initiatives.</p> <p>10.1 Assesses educational needs of staff and provides in-service education when necessary.</p> <p>11.1 Collaborates with the Clinical Nurse Specialists and nursing staff on the development of Patient Care</p> <p>Standards/Critical Pathways involving the care of patients.</p> <p>12.1 Collaborates with the primary nurse in assessing the learning needs of patients and their families and</p> <p>providing education support when needed.</p> <p>13.1 Teaches trachea care to the primary nurse in assessing the readiness of caregivers to initiate track</p> <p>teaching.</p> <p>14.1 Provides written information to patients and their families pertaining to illness or surgical procedures and</p> <p>develops teaching tools as needed.</p> <p>15.1 Conveys information pertaining to hospital policy and procedure to resident staff faculty and other</p> <p>members of the service.</p> <p>Qualifications</p> <p>EDUCATION:</p> <p>Bachelor's Degree in Nursing</p> <p>EXPERIENCE:</p> <p>A minimum of three (3) years experience in inpatient services, ambulatory oncology service or operating room.</p> <p>SPECIAL SKILLS:</p> <p>Knowledge of physiology and disease involving the disease unit specialty and familiarity with tumor staging .</p> <p>Excellent communication skills. Responsible for assisting in pre-op teaching classes held for laryngectomy/supralottic laryngect and composite</p> <p>resections with flap reconstructions:, Clinic procedures for adult and pediatric patients, including tracheotomies and</p> <p>critical path laryngectomy.</p> <p>LICENSURE/CERTIFICATION:</p> <p>Connecticut RN license</p> <p>Additional Information</p> <ul> <li>1-2 years' experience in care coordination or case management. </li><li>Preferred experience or training in palliative care or serious illness in pediatrics. </li><li>Strong skills in organization, communication, and interdisciplinary care. </li></ul> <p>YNHHS Requisition ID</p> <p>148897</p>
