<p>Monday - Friday: Work Schedule (8:00am - 4:30pm)</p> <p>As the Community Partner Coordinator, you serve as a critical link for our patients and New Bedford Community Health ("the Health Center") to assistance offered by community agencies and hospital-based services. By collaborating with the multi-disciplinary teams at the Health Center, you will help patients navigate services that are available to them.</p> <p>The CP Coordinator is also a vital advocate for our patients within the Community Care Cooperative (C3) Accountable Care Organization (ACO) that the Health Center is affiliated with.
