<p>Brookhaven Retirement Community</p> <p>Must be a Registered Nurse</p> <p>Position Summary:</p> <p>Coordinates a resident's skilled care according to physician orders and ensures communication of skilled care with the interdisciplinary team, resident and family.</p> <p>Essential Functions:</p> <ul> <li>Because BrookHaven exists to care for our residents, regular attendance and punctuality are essential to provide quality care </li><li>Leadership: Ability to take action and achieve results through others </li><li>Commitment to BrookHaven mission, values and philosophy and Service Excellence Initiative </li><li>Must be able to complete performance requirements independently while using good judgment as well as have the ability to harmoniously work with others </li><li>Must be able to read, write and understand the English language </li></ul> <p>Position Responsibilities:</p> <ul> <li>Must follow BrookHaven Policies and Procedures </li><li>Must meet job related competencies as outlined in the Skills Competency Manual </li><li>Pre Admission: Conducts on-site assessments, verifies payer source, completes pre admission screening form, pharmacy and med surge quotes </li><li>Admission: Review and initiate orders, communicate with care community personnel, greet resident and family </li><li>Coordinate residents MDS's and Care Conferences </li><li>Participates in the QA process and "Nurse On-call" program </li><li>On-going communication regarding resident goals, progress and discharge planning </li><li>Assumes responsibility for maintaining his/her own professional competence </li><li>Oversees skilled residents care and works closely with the Office Manager </li><li>Perform other duties as assigned </li></ul> <p>Knowledge, Skills and Abilities:</p> <ul> <li>Graduation from an accredited school of Nursing. </li><li>Current license as a Registered Nurse with the State Board of Nursing. </li><li>CPR certification recommended. </li></ul>
