<p>Salary/Pay Rate:$50,000-$64,000/yearPosted Date:04/03/2025</p> <p>Richard Hall Community Health & Wellness Center</p> <p>Hours: 8:30 am-5:00 pm; one day/week will work evening shift (until 8 pm)</p> <p>Position Summary</p> <p>Assist agency prescribers, nurses and staff providing person-centered, trauma informed, wellness and recovery case management services to medication clinic individual of all ages. Communicate with other health care providers to collaborate and coordinate physical and mental health services. Provide medication education and nutritional counselling in accordance with standard guidelines. Assess health needs, develop, and implement appropriate plan of intervention. Assist agency prescribers in developing treatment plan for individuals serve by agency in person centered, trauma-informed, wellness and recovery-oriented goals. Assess and facilitate release of information and provide collaborative care to all clients, work with guardians to update treatment information and treatment plan.</p> <p>Essential Functions</p> <ul> <li>Face to face contact with clients: 20 hours per week. </li><li>Lab coordination, collection of urine samples, monitoring walk-in clinic and other relevant activities and follow-up paperwork accounts: 10 hours per week. </li><li>PCP Groups: 4 hour per week or as needed </li><li>Phone contact: approximately 20 hrs./wk. </li><li>Med group: 1 hour per week or as needed </li><li>Medication and nutritional teaching: 8 hours per week. </li><li>Any and all staff may be asked to provide clinical coverage as needed and as appropriate based on the needs of the Center. </li><li>Collection of selected data about the health status of the client such as vitals, height, weight, and tracking lab results. </li><li>Participate in the development of the plan of care with prescribers & other team members and safely implement the plan. </li><li>Interact with other support staff to ensure continuity and coordination of care. </li><li>Respond to crisis and interventions as needed. </li><li>Assess for serious medication side effects, reports, and documents. </li><li>Facilitates a wellness group which includes preventive health and nutritional education. </li><li>Assesses and triage phone inquiry by clients. </li><li>Assist prescribers with obtaining release of information and all aspects of care. </li><li>Assesses general mental status and reports to psychiatrist. </li><li>Case management includes linking with several key agencies serving individual of all ages. </li><li>Participate in supervision to optimize effectiveness and productivity. </li><li>Consults with other internal and external providers to assure coordination of care. </li><li>Document case management activities in a timely and effective manner and in accordance to agency/state/accrediting body standards. </li><li>Communicate pertinent information regarding individuals served to agency prescribers, nurses, therapists, and staff. </li><li>Coordinate care with other health care provider as requested by prescribers. </li><li>Engages in continuing education to maintain and develop skills to meet agency competency standards. </li><li>Participates in department and agency wide meetings designed to assure program effectiveness. </li><li>Works collaboratively with all Center staff. </li><li>Preforms other duties as assigned by management. </li></ul> <p>Essential Qualifications</p> <ul> <li> <p>Minimum of 1 years' experience in psychiatric nursing or any equivalent combination of education, training or experience.</p> </li><li> <p>Experience in medical nursing is required.</p> </li><li> <p>BCLS Certification</p> </li><li> <p>Maintain current CPR for health care providers.</p> </li><li> <p>Maintain current State of New Jersey LPN Licensure.</p> </li><li> <p>Current certification as a Licensed Practical Nurse in the State of New Jersey.</p> </li><li> <p>Possession of a valid NJ driver's license.</p> </li><li> <p>Age Specific Knowledge & Skills: Must be able to demonstrate the knowledge and skills necessary to provide care to:</p> </li><li> <p>Children</p> </li><li> <p>Adolescents</p> </li><li> <p>Adults</p> </li><li> <p>Older Adults</p> </li><li> <p>Must demonstrate knowledge of key principles of person-centered, trauma informed, wellness and recovery delivery of services, growth and development of the life span, possesses the ability to assess data reflective of the individual's status and be able to interpret the appropriate information needed to identify each individual's needs relative to his/her age in accordance with the Center's policies and procedures.</p> </li><li> <p>Ability to administer medications as directed.</p> </li><li> <p>Ability to take vital signs.</p> </li><li> <p>Ability to maintained strict infection control protocol as per standard guidelines.</p> </li><li> <p>Crisis stabilization skills including ability to assess individual's need for emergency medical and psychiatric screening.</p> </li></ul> <p>Preferred Qualifications</p> <ul> <li>Experience with mentally ill individuals and families. </li><li>Triage and telephone skills. </li><li>Confidentiality is strictly upheld. </li><li>Knowledge of the principles and skills needed for practical nursing to provide care and treatment. </li><li>Knowledge of medications and their effect on clients. </li><li>Knowledge of physical examinations and diagnostic tests. </li><li>Knowledge of common safety hazard and precaution to establish a safe work environment </li><li>Lived experience in mental health and/or substance use disorders and in recovery. </li></ul>
