Partners with the patient, family, healthcare team, and community agencies in the safe transition of care to the next most appropriate level. Must be able to communicate effectively with physicians, staff, patients and families and community organizations. The social worker must be able to handle multiple priorities and have good organizational skills.</p> <p>Duties include:</p> <ul> <li>Performs psychosocial assessments and provides accurate community resource linkage as part of discharge plan, identifying correct resources, educating patient on use of resources and completing all requirements of successful referral. </li><li>Employs a range of clinical interventions such as crisis intervention, care coordination, information gathering and referrals.
