<p>The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient-centered hospice that maximizes resources at bedside while being operated locally. Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve. When end-of-life care is necessary, patients and their families are in their most vulnerable time. We don't require high caseloads or unmanageable quotas, which gives our team more time with each of their patients. Our top priority is our patients and the people serving them.</p> <p>Schedule: PRN hours between the hours of 8a-5p Monday- Friday</p> <p>What We offer:</p> <ul> <li>Flexible Hours </li><li>Great culture and team atmosphere </li><li>Mileage Reimbursement </li><li>Employee Referral Program </li><li>Employee Discount Programs </li></ul> <p>What You'll Do:</p> <ul> <li>Performs Licensed Massage Therapy assessments, diagnostic test, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care. </li><li>Develops a massage therapy care plan to provide direction to other health team members involved in the plan of care. Reviews and updates per Agency policy and client need. </li><li>Documents all findings, plans, interventions, and outcomes and submits documentation to the hospice office within a timely manner. </li><li>Communicates plans and changes to the physician and to the client's Clinical Team Lead and other caregivers through the plan of care, progress notes and participation in care conferences. </li><li>Communicates changes in schedule and physician orders to the office on the day the changes are made. </li><li>Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assists team members as needed to ensure that this level of service is consistently provided. </li></ul> <p>Qualifications:</p> <ul> <li>Graduate of a Licensed Massage Therapy Program. </li><li>Has a current license/registration as a Licensed Massage Therapist. </li><li>Minimum of two (2) years' experience in health care field. </li><li>Hospice experience preferred. </li><li>Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. </li><li>Shows ability to organize and prioritize workload independently. </li></ul> <p>Text 8521 to (937)863-9487 to apply</p> <p>#ACHOS</p>
