Read more about us at venicefamilyclinic.org.</p> <p>Make a difference in the community with our Venice Family Clinic as a Housing Case Manager. With guidance and supervision from the Case Management Supervisor, the Housing Case Manager provides high quality case management to medically complex patients experiencing homelessness. This position provides direct housing navigation and stability support in both field-based and clinic settings for patients eligible for the Community Supports housing program.</p> <p>Salary range: $30.53 - $40.30 Hourly</p> <p>Job Qualifications</p> <p>Press space or enter keys to toggle section visibility</p> <p>Required:</p> <ul> <li>Minimum of one year experience in homeless service case management. </li><li>Knowledge of community resources, housing first methods, ICMS, tenant advocacy, harm reduction, trauma informed care, motivational interviewing and general clinical knowledge. </li><li>Must maintain a valid California Driver's License and auto insurance in good standing; ability to operate both personal and company owned vehicles to travel between worksites and other locations as required. </li><li>Ability to create and maintain boundaries with clients </li><li>Ability to follow through on assignments, complete required program documentation, and skill in managing multiple deadlines and demands. </li><li>Ability to work independently and as part of a group, and skill in maintaining cooperative working relationships. </li><li>Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality. </li><li>Ability to work effectively with people of diverse cultural and educational backgrounds. </li><li>Skill in creating flyers and other program-promoting documents. </li></ul> <p>Position is subject to the California DMV's "Pull Notice System" and continued employment is contingent upon proof of a satisfactory driving record.</p> <p>As a condition of employment, the final candidate who accepts a conditional offer of employment will be required to disclose if they have been subject to any final administrative or judicial decisions within the last seven years determining that they committed any misconduct; received notice of any allegations or are currently the subject of any administrative or disciplinary proceedings involving misconduct; have left a position after receiving notice of allegations or while under investigation in an administrative or disciplinary proceeding involving misconduct; or have filed an appeal of a finding of misconduct with a previous employer.</p>
