INCOMPLETE APPLICATIONS & RESUMES RECEIVED IN LIEU OF APPLICATIONS WILL NOT BE CONSIDERED.</p> <p>SUMMARY: The purpose of a SWAP Coordinator is to support and maintain pre-employment transition services as well as an employment program for youth with mild/moderate disabilities ages 15 through 24. The SWAP Coordinator works directly with the District and the Colorado Division of Vocational Rehabilitation to provide services that result in training and successful competitive employment for eligible youth.</p> <p>MINIMUM QUALIFICATIONS:</p> <ul> <li>Bachelor's degree in Special Education, Regular Education, Social Services, Vocational or related field </li><li>Valid Colorado Driver's License </li><li>Ability to work with individuals with disabilities in obtaining and maintaining meaningful employment </li><li>Demonstrated ability as a liaison/resource with families, community service agencies, businesses, potential employers, educational staff and the Division of Vocational Rehabilitation regarding career and community programming for eligible youth </li><li>Ability to supervise and direct an assistant </li><li>Excellent oral and written communication skills and the ability to work collaboratively with youth with disabilities, school and community agency personnel </li><li>Strong interpersonal relationship skills </li><li>Experience working with diverse cultures and backgrounds desirable </li></ul> <p>JOB RESPONSIBILITIES:</p> <ul> <li>Identify and place appropriate students in coordination with special educators and vocational rehabilitation personnel </li><li>Coordinate services such as: vocational assessment, career planning, counseling on postsecondary educational program opportunities, job development and placement, case management </li><li>Supervise SWAP Specialist(s) (under the direction of the Director of Special Education or designee) </li><li>Ensure compliance with all SWAP rules, regulations, and expectations </li><li>Compile and maintain accurate records, prepare required state reports, prepare requests for continued funding, etc. </li><li>Provide employment-related training, including, but not limited to, daily living skills, social skills, self-advocacy and community access and job-seeking/maintenance skills </li><li>Provide crisis intervention, follow-up support and counseling for families and eligible youth </li><li>Provide direct support to employers and participants to improve job success, including, but not limited to; job accommodations, job coaching, advocacy and counseling </li><li>Develop and support career plans leading to competitive employment for eligible youth </li><li>Secure and coordinate work experiences and job placements for SWAP participants </li><li>Provide staff development and training regarding SWAP in the schools and community </li><li>Maintain close communication with the Director of Special Education and/or designee and perform other duties as assigned </li></ul> <p>Westminster Public Schools is an Equal Opportunity Employer.
