<p>The Registered Nurse - Home Based Primary Care will deliver holistic care through collaborative strategies with others in the assigned area. They provide comprehensive nursing care for the patient in the Home Based Primary Care program.</p> <p>The Home Based Primary Care Nurse focuses on problems, initiates referrals to other disciplines and/or services as appropriate. Care is coordinated across the continuum of care. They accurately document in the medical record patient findings, assessments, and care provided and effectively function independently as contributing member of an interdisciplinary team.</p> <p>Major Duties include, but are not limited to:</p> <ul> <li>Determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. </li><li>Formulates a plan of care based on nursing assessments </li><li>Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. </li><li>Responsible for providing competent, evidence-based care to assigned patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. </li><li>Assists in directing the provision of nursing education/orientation/competencies and providing quality improvement and outcomes utilization consultation. </li><li>Demonstrates and/or verbalizes correct responses to emergencies as well as initiates/appropriate action in emergencies. </li><li>Provides education to the veteran and caregiver about developing an individual emergency preparedness plan. </li><li>Initiates and utilizes positive, effective communication skills, and professional behaviors that promote cooperation, teamwork and effective interaction with internal and external customers. </li><li>Accurately documents in the medical record patient findings, assessments, and care provided. </li><li>Demonstrates understanding of infection control precautions appropriate for the environment of care. </li><li>All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service. </li></ul> <p>VA offers a comprehensive total rewards package: VA Nurse Total Rewards</p> <p>Pay: Competitive salary, regular salary increases, potential for performance awards</p> <p>Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)</p> <p>Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA</p> <p>Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)</p> <p>Licensure: 1 full and unrestricted license from any US State or territory</p> <p>Work Schedule: Full-Time, Day shift schedule.
