We learn, grow, and achieve more - in our careers and in our communities.</p> <p>Job Description Summary:</p> <p>The Care Manager works in partnership with patients, families, caregivers and providers to deliver targeted care that is organized, coordinated, cost effective and patient-centered. The Care Manager collaborates with interdisciplinary staff and stakeholders within the care continuum to promote patients' optimal health through disease prevention and management strategies. Evidence-based interventions are applied to populations for cost efficient and outcome-based care. Outcomes include disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates.</p> <p>Responsibilities And Duties:</p> <p>Minimum Qualifications:</p> <p>Bachelor's Degree (Required)CPR - Cardiopulmonary Resuscitation - American Red Cross</p> <p>Additional Job Description:</p> <p>Must be a graduate of an accredited school of nursing. Must be currently licensed as a Registered Nurse in the State of Ohio and have a current state driver's license. Driving record must be acceptable to the Hospital's current liability carrier for all Community Case Managers. Must have the knowledge and skills that would generally be acquired through three to five years of clinical Experience. Community or Home Health Care nursing Experience for the Community Case Manager. Work requires the ability to analyze and solve complex problems requiring the use of basic scientific clinical, or technical principles plus in-depth, Experience -based knowledge. Work requires excellent communication skills in order to interact in a professional manner with patients, administration, visitors and physicians concerning immediate case issues, policies and procedures, committees, training, etc. Knowledge, skill and ability to provide care for neonatal, pediatric, adolescent, adult and geriatric patient population. Work is not completely standardized. Assignments are often received in the form of results expected, due dates and general procedures to follow.
