<p>Position Summary:</p> <p>The Sepsis Coordinator is responsible for the development, implementation, and ongoing evaluation of Griffin Health's sepsis care initiatives. Serving as a clinical expert, educator, and performance improvement leader, the coordinator will work across departments to reduce sepsis-related mortality and enhance outcomes through early intervention and standardized care.</p> <p>Key Responsibilities:</p> <ul> <li> <p>Lead and coordinate the hospital's sepsis program, implementing best practices in line with current clinical guidelines.</p> </li><li> <p>Monitor and analyze SEP-1 performance data; identify trends and improvement opportunities.</p> </li><li> <p>Conduct chart reviews and ensure documentation aligns with CMS measure requirements.</p> </li><li> <p>Collaborate with nursing, quality, infection prevention, case management, and leadership teams.</p> </li><li> <p>Provide ongoing education and support to clinical staff on sepsis recognition and treatment protocols.</p> </li><li> <p>Participate in root cause analyses and case reviews of sepsis-related mortality or bundle noncompliance.</p> </li><li> <p>Stay current on national sepsis care standards and regulatory updates.</p> </li><li> <p>Assist with EMR optimization, including sepsis alerts, order sets, and documentation workflows.</p> </li><li> <p>Present data and improvement plans to committees and leadership as needed.</p> </li></ul> <p>Qualifications:</p> <p>Required:</p> <ul> <li> <p>Licensed Registered Nurse (RN) or equivalent clinical background</p> </li><li> <p>3-5 years of experience in acute or critical care settings</p> </li><li> <p>Strong knowledge of sepsis management and current clinical guidelines</p> </li><li> <p>Experience with quality improvement and data analysis</p> </li></ul> <p>Preferred:</p> <ul> <li> <p>Bachelor's degree in Nursing or related field</p> </li><li> <p>Experience with SEP-1 measure abstraction or Core Measures</p> </li><li> <p>Familiarity with EMR systems (e.g., Meditech) and data platforms (e.g., Q-App)</p> </li><li> <p>Certified Professional in Healthcare Quality (CPHQ) or similar credential</p> </li></ul> <p>Key Competencies:</p> <ul> <li> <p>Strong leadership and communication skills</p> </li><li> <p>Analytical, detail-oriented, and data-driven</p> </li><li> <p>Collaborative and self-directed</p> </li><li> <p>Skilled educator and advocate for change</p> </li><li> <p>Passion for evidence-based care and continuous quality improvement</p> </li></ul> <p>Why Griffin Health?</p> <p>Griffin Health is a recognized leader in patient-centered, quality-driven care.
