<p>Job Summary:</p> <p>The Providence Center Quality Improvement Outcomes and Feedback Coordinator will provide support for the development, collection, extraction, processing, and reporting of service, consumer, financial and other data maintained in various Center systems and external sources.</p> <p>Duties and Responsibilities:</p> <p>Information Systems</p> <p>Utilize SPSS software to create and revise coding to aggregate results for each patient experience survey question.</p> <p>Test new or modified forms and workflows in Epic.</p> <p>Create and assist with data analysis and develop reports to identify trends and patterns using SPSS, MS Access, MS Excel and RL Datix (Critical) Incident Reporting system.</p> <p>Ensure Quality Improvement information on the TPC Intranet page is current. Archive old content, ensure dating of documents is correct.</p> <p>Configure critical incident reporting system per identified operating unit needs.</p> <p>Collaborate with CNE IT for technical implementation and maintenance of critical incident reporting system.</p> <p>Collaborate with external vendor when critical incident reporting system fails or is not readily available.</p> <p>Serve as internal customer support for critical incident reporting system.</p> <p>Troubleshoot, analyze, identify system issues, provide solution for critical incident reporting system.</p> <p>Configure and maintain templates, roles, scopes, user assignments, alerts and schedules of critical incident reporting system.</p> <p>Design survey templates in survey software to include creating survey answer key and crosswalk table for compiling survey results</p> <p>Data Collection/Reporting/Quality Improvement</p> <p>Support quality improvement team via data collection.</p> <p>Collect multiple forms of data (statistical, direct observation, etc..) from Center programs. Transform gathered data into reports (graphic and narrative).</p> <p>Responsible for the collection, quality checking and analysis of Center clinical and administrative forms and processes that includes template updates, creation of new templates, naming conventions, translating to support DEI mission.</p> <p>Investigate issues in the medical record and update as needed (Ex: BHOLD, episodes, health home requirement, etc.)</p> <p>Maintain data documentation and reporting system (homegrown) of client feedback data.</p> <p>Prepare summary reports of quality data and improvement activities for presentation to internal and external audiences.</p> <p>Analyze data to identify patterns and trends and prepare summary reports for review by managers and senior leaders.</p> <p>Data entry of completed RI OEI surveys into survey software and transfer data to Microsoft Access tables for reporting to BHDDH.</p> <p>After each survey offering & data entry, run SPSS software for each survey and transfer results to Microsoft Excel for formatting, calculating response rates and creating charts.</p> <p>Review Patient Experience client survey results quarterly with department directors and managers.</p> <p>Recommends improvement for the development and modification of clinical templates that require build in the electronic medical records including creating specifications of the forms and coordinating with clinical staff to ensure they meet program needs while maintaining compliance with regulations and standards</p> <p>Identify areas of data collection and reporting that can be used to improve operations and services.</p> <p>Ensure reports are accurate and completed in a timely manner.</p> <p>Serve as liaison for grant initiatives around form development, data collection and reporting.</p> <p>Assist with analysis and evaluation of grant data for internal and external reporting requirements.</p> <p>Patient Experience</p> <p>Serves as a Feedback Coordinator providing the following:</p> <p>Maintains an appropriate data documentation and reporting system of client feedback data. Analyzes data on a periodic basis, and as requested, to identify patterns and trends and prepares summary reports for review by Director and senior leaders.</p> <p>Serves as a resource to department managers and senior leadership to understand client feedback data and develop improvement plans that include published best practices.</p> <p>Coordinates the centers formal client satisfaction survey process, including regular communication of the results and associated analyses.</p> <p>Collaborates with other staff in the development and implementation of educational programs and other proactive strategies to improve client satisfaction.</p> <p>Responsible for the collection, quality checking and analysis of Center clinical and administrative forms and processes that includes template updates, creation of new templates, naming conventions, translating to support DEI mission.</p> <p>Responsible for the creation of survey forms used for data collection, scanning and processing.</p> <p>Maintain data documentation and reporting system (homegrown) of client feedback data.</p> <p>Revise survey tools as requested.</p> <p>Distribute and collect surveys pursuant to survey distribution schedule. Maintain and update schedule as needed.</p> <p>Event Reporting</p> <p>Responsible for daily monitoring and closing out incident reports in the RLDatix event reporting system.</p> <p>Ensure events have manager and/or director level follow up.</p> <p>Communicate with Director of QI to discuss events that require follow up.</p> <p>Report specified events to licensing agency within 24 hours of notification of event.</p> <p>Respond to staff inquiries for assistance with event reporting system.</p> <p>Create, maintain reports and dashboards of critical incident reporting system.</p> <p>Licensing & Accreditation</p> <p>Serves as a resource for licensing agency regulations and accreditation body standards.</p> <p>Create newsletters pertinent to licensing regulations and accreditation standards.</p> <p>Serve as a resource to licensing and accreditation staff when program audits occur.</p> <p>Other Duties</p> <p>Provide coverage for QI Sr Healthcare Data Analyst, Compliance Billing Analyst. Manager of QI and Director of QI as needed.</p> <p>Perform special projects as assigned.</p> <p>Special Projects</p> <p>Assist the Director/Manager with special projects as assigned related to Quality, regulatory compliance, Human Rights, complaints, and Corporate Compliance.</p> <p>Maintain cooperative relationships with other employees, consumers, and the public.</p> <p>Communicate effectively with staff, representatives of other agencies and the public.</p> <p>Work in cooperation with co-workers and supervisory staff.</p> <p>Communicate with co-workers regarding problems in a constructive manner.</p> <p>Accept and use direction and supervision.</p> <p>Benefit from constructive criticism.</p> <p>Have a dedication to teamwork and collaboration.</p> <p>Attend trainings, conferences, and committee meetings.</p> <p>Provide feedback to Director on the content of educational programs and committee meetings attended</p> <p>Attend mandatory in-service trainings and other trainings required.</p> <p>Other duties may be assigned.</p> <p>Requirements:</p> <p>Bachelor's degree (BA) preferred or 3- 5 years experience in behavioral health and performance improvement.</p> <p>Experience in behavioral health and interaction with clients.</p> <p>Must possess excellent written, analytical, and organizational skills, good typing skills required.</p> <p>Computer skills are required, knowledge of PowerPoint, Excel, Word and Access highly desirable.</p> <p>Accreditation standards, State Regulations, Regulatory compliance, Data aggregation and analysis, techniques, Data Entry, Mathematics.</p> <p>Must possess excellent organizational and communication skills and ability to work on Center's computer system. Ability to do queries a plus.
