<p>The School-Based Health Center (SBHC) Community Health Worker/Navigator works as part of a multidisciplinary care team that includes a Nurse Practitioner and Behavioral Health Clinician, collaborating closely with school staff to support the overall health and well-being of students.</p> <p>Work Location and Schedule:</p> <p>This position is based at Claremont/Woodland Academy School, 15 Claremont Street, Worcester, MA, 01610 within the Worcester Public Schools. Regular hours are Monday through Friday, 7:30am to 3:30pm School-Based Health Center (SBHC) assignments may vary based on program and patient care needs.</p> <p>Position Summary:</p> <p>The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services through FHCW and within the community, and make and follow-up on referrals for external services.</p> <p>Program Specific Essential Duties and Responsibilities:</p> <p>Supports the care team with patient follow-up, reminders, and addresses barriers to care.</p> <p>Documents all services as required and assists in meeting all program requirements.</p> <p>Collects and analyzes data related to the project and works with data management staff for data collection.</p> <p>Provides outreach and meets with patients to explain services and coordinates the treatment plan.</p> <p>Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable.</p> <p>Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.</p> <p>Any and all other duties as assigned.</p> <p>Qualifications and Education Requirements:</p> <p>One of the following combinations of education and employment experience must be met in order to be considered for the position:</p> <p>Education and Experience:</p> <ul> <li>High School Diploma or GED required bachelor's degree in a related discipline preferred. </li><li>Preferred candidate will have Community Health Worker or Medical Interpreter certificate of completion. </li><li>Minimum of two years relevant work or college-level internship experience required. </li><li>Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners. </li></ul> <p>Bilingual English or another language appropriate to the patient population required.</p> <p>Local travel required for home visits to patients.</p> <p>Community Health Worker Core Training required within first year of employment.</p> <p>Flexible evening work and weekend hours are required to meet FHCW community or patient needs.</p> <p>Reporting to:</p> <p>Director of School Based Health Centers</p> <p>EOE</p> <p>Monday-Friday; 7:30am -3:30pm</p>
