<p>PRIMARY CARE NURSE</p> <p>Do you want to utilize assessment and teaching skills to foster patient health?</p> <ul> <li>Are you looking for benefits effective on the first day of employment? </li><li>Do you want to take advantage our tuition assistance program? </li><li>Are you someone that can respond to the health education needs of patients and families? </li><li>Do you want to work where your professional growth and development are a priority? </li><li>Are you able to assure clinic flow and that patient needs are met? </li><li>Do you want to provide comprehensive health care of the highest quality to everyone in our community, regardless of ability to pay? </li></ul> <p>In this role you will:</p> <ul> <li>Triage patients by telephone or in person to determine urgency and level of care needed, assuring appropriate care disposition. </li><li>Perform patient procedures, assesses patient health status, and provides health education to patients/families regarding pertinent health issues, clinical conditions, treatments, recommended follow up actions and /or home care. </li><li>Communicate pertinent abnormal test results to provider. </li><li>Track patients for necessarily follow up documenting appropriately. </li><li>Utilize those skills to interact with patients/clients, team members and staff from other departments. </li></ul> <p>The successful applicant will offer:</p> <ul> <li>Graduate of accredited nursing program, current Mass. Licensure, BLS certification. </li><li>Bi-lingual in Spanish /Khmer/Russian with knowledge of medical terminology preferred. </li><li>Previous Community Health or Ambulatory Experience preferred. </li><li>A desire to work as a collaborative member of a multi-disciplinary team. </li></ul> <p>Please apply now and grow with LCHC</p> <p>Thank you!</p>
