With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.</p> <ul> <li>Manages the delivery of high-quality home care services to patients consistent with agency standards, policies, and state and federal regulations by providing leadership and guidance to nursing staff </li><li>Ensures adequate staffing and appropriate patient assignments </li><li>Maintains the integration of interdisciplinary services </li><li>Facilitates the achievement of agency goals related to clinical outcomes, patient experience of care, fiscal responsibility, and staff retention </li></ul> <p>Qualifications:</p> <ul> <li>Graduate from an accredited School of Nursing; BSN preferred. </li><li>Current NYS RN license and registration required. </li><li>Minimum of 3 years' experience in a Certified Home Health Agency; supervisory experience preferred. </li><li>OASIS E; clinical outcome measures. </li><li>Federal Conditions of Participation and NYS DOH regulations related to Home Health Care. </li><li>STAR ratings </li></ul> <p>Apply</p>
