<p>Position#Summary Accountable for assisting consumers in the Emergency services/Screening program including: ensuring the coordination of patient care through follow-up with consumers, Psycho-education, Assisting patients with linkage to treatment, other services # entitlements. Works collaboratively with the Emergency Services staff, 2 Central, psychiatry, Primary care Physicians, HBH and community outpatient treatment programs. Supports the mission, values and philosophy of HMC. Primary Position Responsibilities Assists in the coordination of patient care for those consumers in Emergency Services through providing follow-up. Provides emotional support and psycho-education to patients in the Emergency Service. Provides high quality Case Management and linkage to care. Documents all activities related to patient support and psycho-education. Educates patients in the Construction of Wellness and Recovery Action Plans and Psychiatric Advanced Directives. Assists with the quality assurance projects of the department. Qualifications Minimum Education: Required: Bachelor#s Degree in Social Work, Psychology, or Equivalent Minimum Years of Experience (Amount, Type and Variation): Required: Personal experience of Mental Health Issues License, Registry or Certification: Required: Valid Drivers License Knowledge, Skills and/or Abilities: Required: Excellent interpersonal communication skills. Ability to identify and resolve problems.
