Coordinates efforts for treatment and promotes quality improvement. Manages educational efforts and compiles data to track outcomes. Makes recommendations for process improvement. Provides referrals and supports follow-up care for patients who are uninsured or underinsured. Facilitates communication between patients and healthcare providers. Ensures continuity of care. Assists patients in navigating the healthcare system.
