Your recruiter will provide the most up-to-date details during the hiring process.</p> <p>How you'll make an impact in this role</p> <ul> <li>Coordinate and oversee the interdisciplinary plan of care from admission through discharge. </li><li>Develop and monitor continuum-of-care plans that incorporate patient and family preferences. </li><li>Advocate for appropriate resources, remove barriers, and collaborate with supervisors and care teams to ensure effective plan implementation. </li><li>Serve as a resource for clinical documentation accuracy, service appropriateness, and post-discharge care planning. </li></ul> <p>What minimum requirements you'll need</p> <p>Licensure / Certification / Registration:</p> <ul> <li> <p>One or more of the following required:</p> </li><li> <p>Clinical Counselor obtained prior to hire date or job transfer date.</p> </li><li> <p>Counselor obtained prior to hire date or job transfer date.</p> </li><li> <p>Social Worker obtained prior to hire date or job transfer date.</p> </li><li> <p>Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.</p> </li><li> <p>Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.</p> </li><li> <p>Case Manager credentialed from the American Case Management Association (ACMA) preferred.</p> </li></ul> <p>Education:</p> <ul> <li>Master's degree in Social Work required. </li></ul> <p>Work Experience:</p> <ul> <li>3 years of experience preferred. </li></ul> <p>What additional preferences we're seeking</p> <ul> <li>LCSW with 1 year experience in home health setting. </li><li>Flexible hours Monday - Friday. </li><li>Charting is done on a tablet using Point Care to be done in the home. </li></ul> <p>Equal employment opportunity employer</p> <p>Ascension provides Equal Employment Opportunities (EEO) to all associates and applicants for employment without regard to race, color, religion, sex/gender, sexual orientation, gender identity or expression, pregnancy, childbirth, and related medical conditions, lactation, breastfeeding, national origin, citizenship, age, disability, genetic information, veteran status, marital status, all as defined by applicable law, and any other legally protected status or characteristic in accordance with applicable federal, state and local laws.
