<p>Additional Information About the Role</p> <p>Join the BJC Hospice team in providing bereavement support for families. You will support our north team in providing virtual, phone and home visits to follow up with families for 13 months.</p> <p>The position will also lead grief support groups and participate in Stepping Stones camp as well as various support groups throughout the year.</p> <p>Monday - Friday position with some evenings and weekends.</p> <p>Overview</p> <p>BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.</p> <p>BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.</p> <p>The BJC Home Care Services Supportive Care staff offer patients with life-limiting illness an opportunity to ease the transition from curative treatment to Hospice care through a unique program. Supportive Care offers a bridge between curative and terminal care.
