YOU Inc. is seeking a Case Manager I to join our Children's Congregate Care team and provide essential support to youth in residential settings.</p> <p>In this role, you'll be part of a multidisciplinary team working to improve the mental health and overall well-being of children and adolescents. This is a great opportunity to gain hands-on experience in behavioral health while making a meaningful difference in the lives of youth and families.</p> <p>If you're ready to build your career in human services and be part of a supportive, mission-driven team, apply today.</p> <p>Responsibilities</p> <p>Case Management & Care Coordination</p> <ul> <li>Provide direct case management services to youth in residential care. </li><li>Support implementation of individualized service and treatment plans. </li><li>Connect youth and families to community resources and natural supports. </li><li>Coordinate care with internal teams and external providers (e.g., medical providers, schools, and state agencies). </li></ul> <p>Discharge & Aftercare Planning</p> <ul> <li>Assist in planning for safe and stable discharge from residential care. </li><li>Coordinate and follow up on aftercare services to support continued success. </li><li>Serve as a liaison between families, treatment teams, and community partners. </li></ul> <p>Team Collaboration & Support</p> <ul> <li>Actively participate in multidisciplinary team meetings and case conferences. </li><li>Communicate updates and concerns in a timely and professional manner. </li><li>Work in the residential milieu as needed to support program operations and youth care. </li></ul> <p>Crisis Response & Safety</p> <ul> <li>Respond to crisis situations and coordinate appropriate interventions. </li><li>Monitor and address safety risks within the program environment. </li><li>Maintain training in CPI/de-escalation, First Aid/CPR, and medication protocols. </li></ul> <p>Documentation & Compliance</p> <ul> <li>Maintain accurate and timely documentation in Electronic Health Records and required systems. </li><li>Ensure compliance with HIPAA, agency policies, and best practices. </li><li>Track services, progress, and outcomes to support quality care. </li></ul> <p>What You Bring</p> <ul> <li>Strong interest in working with youth in behavioral health or residential settings </li><li>Ability to coordinate services and manage multiple priorities </li><li>Strong communication, organization, and problem-solving skills </li><li>Commitment to trauma-informed, strengths-based, and client-centered care </li><li>Ability to work both independently and as part of a team </li></ul> <p>Qualifications</p> <ul> <li>High School Diploma or equivalency certificate. </li><li>Valid driver's license (Class C or D) </li></ul>
