This position is responsible for referral coordination, intake support, recovery-oriented outreach, SUD screening and education, as permitted by credentials, and connection to appropriate treatment and recovery resources.</p> <p>This role requires ongoing relationship-building with statewide Recovery Community Organizations, community partners, referral sources, and other recovery-focused stakeholders to strengthen referral pathways, support recovery engagement, and improve coordination across the recovery services system.</p> <p>The position may be filled at the CRSW or LADC level. Appropriate licensure or certification is required and is not negotiable. Duties, responsibilities, title, and salary will be aligned with the successful candidate's credential and authorized scope of practice.</p> <p>Essential Duties & Responsibilities</p> <ul> <li>Support referrals, intake coordination, access to services, and recovery-oriented engagement as a member of the SUD Services care team. </li><li>Provide statewide outreach, education, and relationship-building with community members, referral sources, Recovery Community Organizations, and other partners regarding Harbor Care services, eligibility, harm reduction, overdose prevention, recovery supports, and access to care. </li><li>Maintain an ongoing and visible presence in statewide Recovery Community Organizations to build trust, strengthen referral pathways, and improve coordination of services. </li><li>Communicate the purpose, scope, and impact of Harbor Care's behavioral health, SUD, and recovery services to clients, residents, partners, and community stakeholders. </li><li>Assist individuals in overcoming barriers to SUD, behavioral health, recovery housing, and related support services. </li><li>Provide SUD screenings and assist individuals who screen positive in obtaining a full SUD assessment and access to treatment when indicated, within credential scope. </li><li>Support individuals in developing recovery goals, aftercare plans, recovery capital, and SUD management plans as appropriate to role and credential. </li><li>Handle phone calls, emails, and inquiries from clients, referral sources, community partners, and others regarding referrals, intakes, and available services. </li><li>Review and process recovery housing applications and intakes, as assigned and within scope of role. </li><li>Coordinate with Harbor Care programs, community partners, service providers, RCOs, recovery housing operators, and multidisciplinary teams to support referrals, wraparound services, care planning, and continuity of services. </li><li>Build and maintain positive working relationships with clients, providers, nurse case managers, agency representatives, supervisors, office staff, recovery home operators, and community stakeholders. </li><li>Document activities, referrals, screenings, intakes, outcomes, waitlists, occupancy, recovery housing participation, and other required data accurately and timely. </li><li>Track and report data across assigned service areas to support reporting, program improvement, funding requirements, and organizational objectives. </li><li>Continuously expand knowledge of community resources, substance use disorders, harm reduction, trauma-informed care, recovery-oriented systems of care, and culturally responsive service delivery. </li><li>Maintain appropriate role definition, professional boundaries, confidentiality, and ethical standards at all times. </li><li>Perform duties in a manner that is respectful of individuals' cultural, religious, ethnic, linguistic, and personal differences. </li><li>Duties and responsibilities may be adjusted to ensure alignment with funding requirements, credential scope, licensure requirements, and organizational objectives. </li><li>Build positive and professional relationships with clients, staff, and community partners </li><li>Maintain accurate and timely documentation, records, or reporting requirements </li><li>Support Harbor Care's philosophy of care, mission, and organizational values in day-to-day work </li></ul> <p>Please note: Duties and responsibilities may evolve based on organizational needs, funding requirements, and program operations.</p> <p>What We're Looking For</p> <p>Required Qualifications</p> <ul> <li>CRSW-Level Responsibilities </li></ul> <p>For candidates hired into a CRSW-level role, responsibilities will focus on recovery support, peer-informed engagement, outreach, education, resource navigation, referral support, recovery planning, and assistance with access to services.</p> <p>CRSW-level duties must remain within the authorized scope of the CRSW credential and may not include clinical assessment, diagnosis, or treatment services requiring LADC or another clinical licensure.</p> <p>LADC-Level Responsibilities</p> <p>For candidates hired into an LADC-level role, responsibilities may include SUD screening, assessment-related functions, counseling, treatment planning support, clinical documentation, and other SUD service activities permitted under the LADC license and Harbor Care policy.</p> <p>Preferred Qualifications</p> <ul> <li>Experience providing community outreach, referral coordination, intake support, recovery support, or related human services work required. </li><li>Experience working with individuals with substance use disorders, behavioral health needs, housing instability, or related barriers to care preferred. </li><li>Experience with data collection, tracking, reporting, and analytics required. </li><li>Experience working collaboratively with community partners, referral sources, RCOs, and multidisciplinary teams preferred. </li><li>Experience working in a multicultural setting preferred. </li><li>At least two years of experience providing community outreach preferred. </li><li>An equivalent combination of education, training, and relevant experience, including military service, internships, volunteer work, and non-traditional career paths, may be considered in meeting position requirements, provided required licensure or certification is met. </li></ul> <p>olunteer work, negotiable. </li><li>Candidates must hold or be eligible to hold by date of hire if approved by Harbor Care, the credential required for the level at which they are hired. </li><li>CRSW-level role: Current Certified Recovery Support Worker credential required. </li><li>LADC-level role: Current Licensed Alcohol and Drug Counselor license required. </li><li>MLADC or other advanced relevant licensure may be considered where applicable. </li><li>Candidate must maintain required licensure or certification in good standing throughout employment. </li><li>Verifiable good driving record and
