Our committed staff of direct support professionals help drive our mission.</p> <p>What We're Looking For:</p> <p>Rock Creek Foundation is seeking a Full Time Psychiatric Rehabilitative Counselor to join our team. The employee counsels disabled individuals to provide vocational rehabilitation services by performing the following duties. The employee is responsible for TX planning, individual crisis management, documentation, case management, and group facilitations, and will work in a multi-disciplinary environment with therapists, psychiatrists and nursing staff.</p> <p>What You'll Do:</p> <ul> <li>Interviews and evaluates disabled applicants, and confers with medical and professional personnel to determine type and degree of disability, eligibility for service, and feasibility of vocational rehabilitation </li><li>Accepts or recommends acceptance of suitable candidates </li><li>Determines suitable job or business consistent with applicant's desires, aptitudes, and physical, mental, and emotional limitations </li><li>Plans and arranges for applicant to study or train for job </li><li>Assesses clients' social, emotional, financial, health, or other problems to develop rehabilitation treatment plan </li><li>Responsible for completing treatment plans as needed with clients, and implements treatment plans with client </li><li>Organizes and conducts support group and vocational groups as needed </li><li>Endeavors to place individual in gainful employment after acquisition of marketable skills </li><li>Resolve work related problems or difficulties, and assigns individual to simpler tasks when worker cannot perform assigned duties </li><li>Observes worker to ensure that work is performed according to prescribed methods and meets established standards </li><li>Maintains working relationships with staff of other agencies and institutions, homes, facilities, and acts as liaison between clients and agency/ institution </li><li>Acts as liaison between clients and their supervisor and co-workers </li><li>Helps clients understand mental illness and how it affects their health, life styles, and employment </li><li>Determines suitable job or business consistent with applicant's desires, aptitudes, and physical, mental, and emotional limitations. Plans and arranges for applicant to study or train for job </li><li>Assists applicant with personal adjustment throughout rehabilitation program </li><li>Aids applicant in obtaining medical and social services during training </li><li>Aids applicants in obtaining additional employment services from DOR's (Division of Rehabilitation Services) </li><li>Responsible for the accurate and timely billing of all services to DOR's and EHR </li><li>Responsible for the accurate and timely completion of authorizations for services in ASO System </li><li>Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required </li><li>Attends mandatory meetings </li><li>Check's work email at the beginning and end of each shift </li><li>Participates in and successfully completes all required trainings </li><li>Other duties as assigned </li></ul> <p>There are no supervisory responsibilities for this position.</p> <p>What We Require:</p> <ul> <li>High School Diploma (or equivalent) and a minimum of two (2) years' experience working with adults diagnosed with varying mental health and intellectual disabilities. </li><li>Knowledge of Internet and Microsoft Office software. </li><li>Must be able to work with "high-risk" consumers who have mental illnesses as well as developmental and physical disabilities.
