In this role you will perform patient evaluation, development and implementation of an appropriate treatment plan, delivery of treatment services, and comprehensive discharge planning.</p> <p>Essential Functions:</p> <ul> <li> <p>Establishes and maintains therapeutic relationship with appropriate interpersonal boundaries</p> </li><li> <p>Manages patient care by first completing an evaluation, establishing a treatment plan with objective and measurable goals (outcomes), and provide direct intervention or supervision of care</p> </li><li> <p>Performs therapeutic procedures (exercise, neuromuscular reeducation, gait, massage, manual therapy, orthotic fitting and training, dynamic activities, compression garments)</p> </li><li> <p>Promotes patient and family rights to self-determination</p> </li><li> <p>Assists family to integrate the medical, social emotional, and family dynamics of the condition into the context of their life situation</p> </li><li> <p>Adjusts patient care schedule and prioritizes tasks as indicated</p> </li><li> <p>Performs documentation per policy and procedures (evaluation, progress notes, discharge summary, and multi-disciplinary education and treatment plans)</p> </li><li> <p>Performs patient and family education regarding interventions, disease/injury, safety, and wellness</p> </li></ul> <p>Additional Job Description:</p> <p>Education / Accreditation / Licensure (required & preferred):</p> <ul> <li>BS/MS/DPT degree in physical therapy required. </li><li>DPT degree preferred. </li><li>Valid PT License Kentucky State Board of Physical Therapy </li></ul> <p>Certification</p> <ul> <li>CPR, BLS through the American Heart Association. </li></ul>
