Provides an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required </li><li>Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided </li><li>Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties </li><li>Provides accurate and timely documentation of patient services to reflect the plan of care </li><li>Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs </li><li>Serves as liaison between patients and families/caregivers and community agencies </li></ul> <p>Qualifications:</p> <ul> <li> <p>Social Work Degree</p> </li><li> <p>Idaho: Bachelor's degree in Social Work; Master's degree preferred.</p> </li><li> <p>Illinois: Bachelor's degree in Social Work; Master's degree preferred.</p> </li><li> <p>Ohio: Bachelor's degree in Social Work; Master's degree preferred.</p> </li><li> <p>The bachelor's degree can be in Psychology, sociology, or other field related to Social Work.</p> </li><li> <p>Oregon: Master's Degree in Social Work required.</p> </li><li> <p>New Mexico: Master's Degree in Social Work required.</p> </li><li> <p>Tennessee: Master's Degree in Social Work required.</p> </li><li> <p>Texas: Bachelor's degree in Social Work; Master's degree preferred.</p> </li><li> <p>Virginia: Bachelor's degree in Social Work; Master's degree preferred.</p> </li><li> <p>Current State Licensure in Social Work</p> </li><li> <p>Idaho: LSW, LMSW, or LCSW</p> </li><li> <p>Illinois: LSW or LCSW</p> </li><li> <p>Ohio: LSW, LISW, or LPSW-S</p> </li><li> <p>Oregon: LMSW or LCSW</p> </li><li> <p>New Mexico: LMSW or LCSW</p> </li><li> <p>Tennessee: LMSW, LCSW or or LAPSW can practice under the supervision of an LCSW</p> </li><li> <p>Texas: LBSW, LMSW, or LCSW</p> </li><li> <p>Virginia: Licensed not required.</p> </li><li> <p>Minimum of one year's experience in health care or hospice setting</p> </li><li> <p>Valid driver's license and proof of insurance is required</p> </li></ul>
