<p>Diakon is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Child, Family, and Community Ministries and Diakon Affordable Housing. General Responsibilities: Provides intensive, strength based, one-on-one support to clients in collaboration with the clinician and reflective of the youth#s mental health treatment plan. Establishes relationships with youth, foster families, biological families, case managers, and other providers in order to deliver comprehensive services Interviews clients, performs observations, and helps patients build a plan to address behaviors. Customize behavioral and safety plans for individual clients. Provides interventions for foster parents and care provider staff. Coordinates care and behavioral planning support with foster parents. Provides feedback to clients, foster parents and care provider staff on behavior plan. Recognize and document critical improvements in client behaviors. Identifies skills, supports and resources that could be used to enhance youths# wellness, including internal and in the community. Assists in the education and supervision of youth in group sessions and activities as needed. Coordinates with other service providers and assists clinical team in building network and resources. Maintains ongoing client documentation as required including progress notes, treatment planning, and assessments. Ability to follow HIPAA guidelines as well as mental health federal # state regulations. Transports and accompanies youth to appointments as assigned. Attends internal and external trainings and applies newly gained knowledge in providing services. Attends and prepares for required court hearings (if applicable). Education/Training: Bachelor#s degree in Social Work, Psychology, or related field of study from an accredited college required. Master#s degree in Social Services or related field of study from an accredited college, preferred. Experience:#A minimum of three (3) years of experience in child welfare field working with children and/or adolescents, strongly preferred. Prior experience working with refugee and/or immigrant populations as well as developing and implementing behavioral or clinical support plans preferred. Bi-lingual English and Spanish required. # ADA: Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 25 lbs.; visual and auditory skills Compensation and Benefits: Diakon offers a competitive compensation and benefits package.
