<p>Department</p> <p>Diabetes & OP Services</p> <p>Job Summary</p> <p>Serves as a nutrition resource for community and physicians. Evaluates, assesses plans, charts in patient's medical record and implements patient's nutritional care. Complies with all nutrition standards of practice. Pursues optimal standards of quality care and customer satisfaction. Maintains Hazard Analysis Critical Control Points (HACCP). Responsible for guiding the education for patients of all ages with Diabetes and their family members on an outpatient basis. Functions as a Diabetes Educator cross-trained in the instruction of all aspects of diabetes.</p> <p>Required Qualifications</p> <ul> <li>Require a Bachelor of Science degree in Dietetics or related field. </li><li>Require one (1) year of teaching or clinical experience in dietetics. </li><li>Require State of Florida Registered Dietitian within six (6) months of employment or within six (6) months after Registered Dietitian status is obtained. </li><li>Require Certified Diabetes Educator (CDE); or CDE eligible to sit for the exam when eligible; or obtains a minimum of fifteen (15) hours of continuing education in Diabetes specific, diabetes related, psychosocial, educational, or program management topics each year. </li><li>Require active membership in professional dietetic associations at a local and national level. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Prefer academic preparation and/or experience in the care of patients with chronic disease and program management. </li></ul> <p>Mandatory Education</p> <p>Preferred Education</p> <p>Required License and Certs</p> <p>Preferred License and Certs</p> <ul> <li>CDCES Certification preferred but not mandatory at time of hire </li><li>Offices at 3 locations: Bee Ridge Road, BlackBurn Point, and Heritage Harbor. </li></ul>
