<p>Role: Licensed Vocational Nurse</p> <p>Reports to: Director</p> <p>Program: 1102- Health and Wellness</p> <p>Department: Health & Wellness</p> <p>Location: Turning Point - 1447 16th Street, Santa Monica, CA 90404</p> <p>Setting: Not 100% Onsite</p> <p>Schedule: Monday - Friday, 8am - 4:30pm</p> <p>Status: Full-time/Non-Exempt/Non-Management</p> <p>Benefits: Medical, Vision, Dental, Life Insurance, 403(b) Retirement plan, Employee Assistance Program (EAP), etc.</p> <p>Openings: 1</p> <p>Job Description Summary:</p> <p>The LVN Advocate will take a leadership role in medication and health issues in the program and will ensure the quality and adequacy of nursing services provided to members.</p> <p>Job Description Essential Duties and Responsibilities:</p> <ul> <li>Complete medical assessment intakes upon admission to Turning Point, Cloverfield, and Village. </li><li>Coordinate PCP appointments or other medical follow ups upon admission. </li><li>Provide medical case management and supportive services to individual members (caseload of 20-30). </li><li>Provide wound care to members as needed. </li><li>Document, chart and file all services provided on nurse charting forms with accuracy and in real time. </li><li>Provide medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens. </li><li>Work in collaboration with the rest of the team- inform NP, case manager, and mental health provider, as appropriate, of any medical and/or medication concerns. </li><li>Maintain medication logs and medication storage in compliance with safety standards and industry standards of care. </li><li>Provide crisis intervention assistance when needed. </li><li>Be able to notify the appropriate community agency in an emergency (i.e., police, paramedics, fire department) </li><li>Obtain vital signs twice a day am/pm times; once a month take weights measurements, and upon intake. </li><li>Help transfer, lift, raise, and turn residents; utilize proper lifting and body mechanics to prevent personal injury </li><li>Provide one-on-one personal care assistance to residents as directed (grooming, feeding, and other support). </li><li>Assist in toileting needs of residents and empty drainage collection devices. </li><li>Assist members to maintain their bedroom areas clean, report to member's advocate any issues. </li><li>Assist with meal service and cleaning of kitchen and dining hall. </li><li>Assist with organizing linen closets, toiletries, nursing areas. </li><li>Maintain open communication with direct supervisor, individual and group supervisor(s). </li><li>Cooperatively and cohesively work with other parties involved in the member's case. </li><li>Participate in bi-weekly staff meetings, bi-weekly nursing meetings, weekly individual and monthly supervision. </li><li>Ensure that medical services are reflective of the agency's treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation. </li><li>Conduct Health/Wellness and medication management groups. </li><li>Maintain confidentiality of member files per HIPAA and all applicable guidelines. </li><li>Other duties as assigned. </li></ul> <p>Job Description Qualifications:</p> <ul> <li> <p>Current, California license as a Licensed Vocational Nurse with a clear license.</p> </li><li> <p>Minimum one years' experience working with individuals experiencing chronic homelessness, mental health, and substance abuse issues.</p> </li><li> <p>Able to physically lift, move, and position members as needed.</p> </li><li> <p>Able to demonstrate acceptance toward members who may have poor coping skills and a high degree of cultural sensitivity to their needs.</p> </li><li> <p>Experience completing medication logs to accepted industry standards.</p> </li><li> <p>Proficient in Microsoft Word, Outlook, Excel, and database applications.</p> </li><li> <p>Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.</p> </li><li> <p>Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision.</p> </li><li> <p>Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.</p> </li><li> <p>Able to obtain and maintain CPR/First Aid certification</p> </li></ul> <p>Job Description Preferred Qualifications:</p> <ul> <li> <p>Experience working with those living with mental illness and/or substance addictions.</p> </li><li> <p>Bilingual Spanish preferred.</p> </li><li> <p>Minimum 35 WPM typing speed preferred.</p> </li></ul> <p>Job Description Work Environment:</p> <ul> <li>Combination of field and office environment. </li><li>Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs. </li><li>On occasion walk or drive to different local sites. </li><li>Will necessitate working in busy and loud environments. </li><li>Will be exposed to elements like cold, heat, dust, noise and odor. </li><li>May need to bend, stoop, twist, and sit throughout the day. </li></ul>
