Apply now and help us help individuals we serve reach their full potential!</p> <p>www.lucasdd.org</p> <p>Community Connections Trainer</p> <p>Develops opportunities for individual(s) and/or group(s) to connect to their community through assessment and exploration of areas of ability, interest, and skill.</p> <p>Connects individuals to community settings and provides support in the setting including specific skill training, adjustment to the environment, identifying and developing natural supports, and modeling appropriate behavior, work skills and social interaction.</p> <p>Facilitates interaction and/or communication between individual(s) and other community members in order to enhance integration.</p> <p>Utilizes fading techniques to withdraw staff and support services when appropriate.</p> <p>Assists with planning, developing, promoting, and implementing recreational activities, Special Olympics sports/league, recruitment of volunteers, travel training, as well as other events and activities that further community integration and employment for the individuals we support.</p> <p>Establishes and maintains collaborative relationships that include supporting and consulting with community members, employers, providers, volunteers, other professionals, families, and residential staff.</p> <p>Complies with all applicable rules, regulations, laws and Board policies and procedures.</p> <p>Required to transport individuals supported by the Board and drive an agency vehicle.</p> <p>Works as a member of an interdisciplinary team to review desired outcomes and action steps as well as identify barriers to community integration, community employment including individual adaptive behaviors, parent and/or caretaker concerns, transportation possibilities, the individual's expressed willingness to participate and other relevant factors.</p> <p>Minimum qualifications</p> <ul> <li>High school diploma or GED </li><li>One (1) year teaching or training experience within one of the following: 1) a community based setting 2) an integrated volunteer worksite 3) school setting, and at least one (1) year of experience working with individuals with developmental disabilities required. </li><li>Valid Driver's license </li></ul> <p>Our organization is committed to maintaining a safe and productive work environment.
