<p>The Caregiver Support Program Social Work Case Manager is responsible for promoting the health and well-being of family caregivers who care for our nation's Veterans, through education, resources, support, and services through evidenced based interventions. The Social Work Case Manager collaborates with Caregiver Support Team to ensure compliance with local and VA Central Office Caregiver Support Program initiatives.</p> <p>Salary Range</p> <p>GS-09 $67,111-$87,249</p> <p>GS-11 $81,198-$105,563</p> <p>Major duties include but not limited to:</p> <p>Clinical Functions</p> <ul> <li>Works independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems using his/her educational training, clinical experience, VA policies and accreditation standards in administering direct services. </li><li>Demonstrate a high level of skill and knowledge of Veteran treatment and disposition in accordance with criteria for care planning across the Veteran care continuum and in a variety of clinical settings and levels of care. </li><li>Demonstrates experience in making rapid assessments and developing crisis management plans for deferral of or admission or to mental health inpatient programs. </li><li>Maintains a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects and medical terminology. </li><li>Facilitate community placements through collaboration with Veterans and their families or caregivers as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner. </li><li>Communicate effectively, both orally and in writing with people from varied backgrounds. Demonstrates effective communication skills that is concise, professional, and accurate. </li></ul> <p>Administrative Functions</p> <ul> <li>Attend all appropriate staff meetings (i.e. team meetings, social work staff meetings, social work supervision meetings, committee meetings, workgroups, etc., as appropriate) and perform other duties as required. </li><li>Participate in activities that obtain feedback from Veterans and work to enhance services as needed. </li><li>Serve on committees and work groups at the facility as deemed appropriate by the supervisor and Chief of Social Work. </li><li>Participate in the monthly Peer Review process. </li><li>Maintain a level of productivity and quality consistent with the complexity of the assignment and with Social Work, accreditation, and facility standards. </li><li>Performs other duties as assigned. </li></ul> <p>Work Schedule: Monday-Friday; 8:00am to 4:30pm</p> <p>Telework: Not Available</p> <p>Virtual: This is not a virtual position.</p> <p>Functional Statement #: GS-9 00341F/GS-11 00342F</p> <p>Relocation/Recruitment Incentives: Not Authorized</p> <p>Permanent Change of Station (PCS): Not Authorized</p>
