Create personalize care plans, assess patient needs, and coordinate with other healthcare providers to ensure a seamless and effective transition with the goal of reducing Emergency Room readmission.</p> <p>These navigators would provide post-discharge follow-up, education, and care coordination via phone or digital platforms, helping to bridge the gap between ED care and long-term management.
