<p>Community Program Liaison Behavioral Health Services - Comprehensive Community Services Hourly Pay Range: $26.88 - $40.31</p> <p>Closing Date: 5/17/2026 or upon receipt of the first 50 applications</p> <p>Bilingual Spanish Speaking Candidates are Encouraged to Apply</p> <p>Job Summary: The Community Program Liaison aides in the enrollment process, monitors quality, and provides support to contracted agencies delivering Comprehensive Community Services (CCS). This role ensures individuals receive appropriate and high-quality care, offers support to care coordination teams, and conducts audits to ensure compliance with DHS 36, ForwardHealth, and Milwaukee County policies and procedures.</p> <p>Essential Responsibilities:</p> <p>Ensure timely and equitable access to Comprehensive Community Services (CCS) through active monitoring of referrals and enrollment processes. This includes coordinating with care coordination teams and internal staff to ensure enrollment. Conduct routine audits and reviews to ensure compliance with DHS 36, ForwardHealth requirements, and Milwaukee County policies and procedures. Provide guidance, training, and technical assistance to care coordination teams and service providers. Provide client contact and clinical support by offering telephonic and walk-in assistance to clients and natural supports engaged in or seeking program services. Participate in clinical staffings and collaborate with system partners also serving the client base, as needed. Perform all other duties as assigned.</p> <p>Minimum Requirements:</p> <p>Current resident of Wisconsin. Possession of a valid Wisconsin Driver's License, able to be maintained throughout employment.</p> <p>Possession of a Bachelor's Degree or higher in Social Work, Psychology, Human Services, or a Related Field. At least three (3) years of professional experience with mental health or substance use disorders. Willing and able to travel within the community up to 5% of the time in this role.</p> <p>Preferred Qualifications:</p> <p>Knowledge of federal, state, and county regulations in relation to the provision of behavioral health services. Understanding of supportive services, including CCS, Community Support Programs (CSP), Targeted Case Management (TCM), and substance abuse treatment programs. Bilingual (English and Spanish Speaking).</p> <p>NOTE: This ORIGINAL APPLICATION is open to qualified residents of the State of Wisconsin at the time of application.
