Assumes leadership role in planning for the program including but not limited to personnel, performance improvement, patient safety, regulatory compliance, programs/services, and physical plant issues as appropriate. The intent of this job description is to provide a summary of the major duties and responsibilities of this position and shall not be considered as a detailed description of all the work requirements that may be inherent in the position.</p> <p>JOB DUTIES AND RESPONSIBILITIES:</p> <ul> <li> <p>Assumes 24-hour responsibility/accountability for planning, organizing and supervising care necessary to assure patient safety and meet the needs of patients served by assigned program.</p> </li><li> <p>Performs direct administrative functions for assigned program.</p> </li><li> <p>Develops, implements and evaluates short and long range performance improvement and operational goals and objectives for the department(s).</p> </li><li> <p>Implements policies, procedures, rules, regulations and recommended practices and ensure that clinical practice standards are met.</p> </li><li> <p>Analyzes and responds accordingly to program specific performance, outcome and financial data.</p> </li><li> <p>Develops capital and operational budgets in collaboration with VP of Patient Care Services Home Health and Finance.</p> </li><li> <p>Participates in staff selection and termination.</p> </li><li> <p>Evaluates current and future staffing based upon patient care needs and initiates adjustments as needed; manages costs within budget.</p> </li><li> <p>Maintains departmental records for administrative and regulatory purposes.</p> </li><li> <p>Participates in entity and network councils; facilitates work teams as appropriate.</p> </li><li> <p>Enhances job knowledge by remaining current in trends in the health care industry and specialty areas.</p> </li><li> <p>Works closely with appropriate management and administrative staff in the development and attainment of program goals.</p> </li><li> <p>Serves as a resource to staff.</p> </li><li> <p>Leads and/or participates in projects involving performance improvement activities.</p> </li><li> <p>Promotes an environment that fosters inquiry, creative thought, professional development and application of evidence-based practice initiatives.</p> </li><li> <p>Reviews performance of staff, assures staff competency and initiates remedial actions, when appropriate. Entity 33 Job Class Code Number 466</p> </li><li> <p>Assists in the development and maintenance of staff competencies.</p> </li><li> <p>Organizes monthly committee/staff meetings as appropriate.</p> </li><li> <p>Maintains confidentiality of all materials handled within the Network/Entity as well as the proper release of information.</p> </li><li> <p>Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.</p> </li><li> <p>Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external).</p> </li><li> <p>Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.</p> </li><li> <p>Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.</p> </li><li> <p>Complies with Network and departmental policies regarding attendance and dress code.</p> </li><li> <p>Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.</p> </li></ul> <p>PHYSICAL AND SENSORY REQUIREMENTS:</p> <p>Reference from U.S. Department of Labor Definitions 199. Medium Work: Involves exerting 20-50 pounds of force occasionally, or 10 to 25 pounds of force frequently, or an amount greater than negligible and up to 10 pounds constantly to move objects. Constantly standing and walking. Physical demand requirements are in excess of those for Light and Sedentary work as defined by U.S. DOL. Reaching: Frequent. Reaching Above the shoulder: Frequent. Twisting: Frequent. Bending/stooping: Frequent. Repetitive movements of elbows/fingers: Frequent. Repetitive movements of wrists: Frequent. Kneeling/Climbing /Squatting: Occasionally. Good physical and mental health. Neat, clean, free from body odors. Conforms to uniform code. Visual and auditory acuity are required to provide comprehensive care.</p> <p>EDUCATION:</p> <p>Doctoral Degree, master's degree from an accredited physical therapy program. Licensed by the Pennsylvania State Board of Physical Therapy required.</p> <p>TRAINING AND EXPERIENCE:</p> <p>Minimum of three (3) years clinical experience with demonstrated leadership abilities required. Sound clinical knowledge in area to be managed. Strong interpersonal skills. Demonstrate ability in Human Resources and Financial Resource Management. Current BLS certification.
