<p>Job Description</p> <p>Position Summary</p> <ul> <li>The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family. </li><li>The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care.
