<p>Franciscan Health Indianapolis Campus</p> <p>8111 S Emerson Ave Indianapolis, Indiana 46237</p> <p>The Healthy Living Center Intake Coordinator is responsible for assisting in the enrollment process for new Healthy Living Center program participants.
