We provide a continuum of care that includes Medication-Assisted Treatment (MAT), Office-Based Addiction Treatment (OBAT), Intensive Outpatient Programs (IOP), and mobile treatment units. Join us in delivering dedicated care and meaningful recovery support.</p> <p>Full Time Hours:</p> <ul> <li>Monday - Friday: 5:30 AM - 2:00 PM </li><li>Ocassional evenings or Sundays for community outreach events, as needed. </li></ul> <p>Your Job as an LPN Case Manager:</p> <p>The LPN Case Manager provides basic medical care to patients in an outpatient opioid treatment program (OTP) environment. They will maintain standards of professional nursing practice in accordance with clinic policy and procedures, provides for the safe delivery of medication in accordance with the policies and procedures, physicians orders and/or current practice. The LPN Case Manager is responsible for thorough, accurate reporting and documentation of the patients receiving services symptoms, responses and progress. Will maintain a patient case load and assist in the dispensary, as needed.</p> <p>Our Benefits:</p> <ul> <li>Semi-Annual Bonus Program </li><li>Medical, Dental, and Vision insurance </li><li>Competitive 401(k) plan </li><li>Paid vacation and sick time </li><li>STAR LRP Student Loan Forgiveness </li><li>Early morning hours offering a great work/life balance </li><li>Opportunity for growth that is second to none in the industry </li></ul> <p>Additional Benefits of Joining our Nursing Team:</p> <ul> <li>Lower stress clinic/office envirionment </li><li>No heavy lifting of patients/no bedside care </li><li>Stable shifts and schedule (day/morning shifts only) </li><li>Early morning hours + early afternoon end time, with occassional evenings or Sundays for community outreach events. </li><li>Be a part of helping others in a recovery environment </li><li>Paid training </li></ul> <p>Your Responsibilities as an LPN Case Manager:</p> <ul> <li>LPN Case Managers will maintain an active caseload and will assist patients with coordinating appointments and provide community resources and support, as needed. </li><li>Provides nursing care and services within the standards and scope of nursing practice, as defined within the state Nurse Practice Act regulations, policies, procedures and protocols as well as clinic and corporate policies and procedures; may assist in the dispensary as needed. </li><li>Nursing care performed is carried out in conformance with established standards and nursing policies, Physician care ordered is carried out and documented appropriately; </li><li>LPN Case Managers will update patient's status on an ongoing basis and document pertinent information obtained; </li><li>Notifies physician of significant changes in status such as difficulties with detox, medical emergency, or change in medical status regarding response to medications, etc.; </li><li>LPN Case Manager will assure nursing treatment plans are developed specific to assessed needs; and initial treatment plans are completed upon admission; </li><li>LPN Case Manager will demonstrates knowledge and utilization of universal precautions in providing direct patient; </li><li>LPN Case Manager when dispensing will Accurately administer medications per ordered note, will document medication administration, & maintain accurate count of controlled substance medication administration on required documents; </li><li>Will assist with Health Homes Enrollment and admission process efficiently and accurately; </li><li>Responsible for completing discharge process efficiently and accurately, as directed. </li><li>Works within a team setting along with the RN Case Manager and Health Homes Coordinator (supervisor). </li><li>Will maintain compliance with all state and federal laws/regulations, DEA and CARF </li></ul> <p>Compensation for roles at Woonsocket Comprehensive Treatment Center varies depending on a wide array of factors including but not limited to the specific location, role, skill set, and level of experience. Woonsocket Comprehensive Treatment Center provides a reasonable hourly range of compensation for roles that may be hired in Rhode Island as set forth below.Role Location: Rhode Island</p> <p>Calculated Salary Range for the role: $25.00 - $30.00 per hour</p> <p>Qualifications</p> <p>Your Education, Skills and Qualifications:</p> <ul> <li>Graduate from an accredited school of nursing </li><li>Case Management and care coordination experience, preferred. </li><li>1 year nursing experience, required.
