Join our team as a Wellness Coordinator, where your outreach and care coordination will directly support patients in accessing vital preventive services and improving their overall well-being.</p> <p>What You'll Do</p> <ul> <li>Be the voice of wellness: Make outreach calls to patients, guiding them through eligible preventive care services like Medicare Annual Wellness Visits. </li><li>Empower healthier choices: Educate patients on the importance of preventive care and help close care gaps that impact long-term health. </li><li>Collaborate with care teams: Work side-by-side with physicians, nurses, and staff to create a seamless, integrated approach to patient care. </li><li>Drive quality improvement: Support primary care practices in achieving better outcomes through documentation, process enhancements, and occasional presentations at staff meetings. </li><li>Support care management: Assist with general office tasks, secretarial duties, and population health initiatives that make a real difference in our communities. </li></ul> <p>What We're Looking For</p> <ul> <li>A positive attitude and a team-first mindset. </li><li>Strong communication skills-especially over the phone and via text. </li><li>Comfort with technology, including Microsoft Office and electronic medical records. </li><li>Ability to multi-task, prioritize, and stay organized in a fast-paced environment. </li><li>A basic understanding of medical terminology is a plus. </li></ul> <p>Qualifications</p> <ul> <li>Education: High school diploma or equivalent required; Associate Degree preferred. </li><li>Experience: Prior clerical experience in a healthcare setting is preferred. </li></ul> <p>This role is ideal for someone who thrives on connection, wants to make a meaningful impact, and enjoys being part of a collaborative care team.
