This is subject to change due to the BHA's business needs.</p> <p>8-5 Monday through Friday</p> <p>Please note: These working arrangements are subject to change.</p> <p>This position provides care coordination to psychiatric hospitals, the justice system, and the Office of Civil and Forensic Mental Health (OCFMH) Emergency Medical Services Facilities (EMS Facilities) when they are working with individuals on emergency mental health holds that may expire prior to finding appropriate placement for evaluation and treatment. As a Social Work/Counselor III this position provides care coordination primarily for individuals under Article 65 of C.R.S. 27-65-108, Care Coordination for persons certified or in need of ongoing treatment. (1) A facility designated by the commissioner shall notify and engage the BHA prior to terminating or transferring a person certified pursuant to sections 27-65-109, 27-65-110, or 27-65-111. The BHA may provide care coordination services to support a person whose certification is terminated but who is in need of ongoing treatment services. (2) The BHA shall, directly or through contract, provide care coordination services to a person certified pursuant to section 27-65-109, 27-65-110, or 27-65-111 and determined by the designated facility and the BHA to need care coordination services. </p> <p>Care coordination for the identified priority populations served by the BHA is also part of this position. Collaboration across the BHA, CDHS Office of Civil Forensic Mental Health, Behavioral Health Administration Service Organizations, and between state agencies such as Health Care Policy and Finance is necessary to ensure individuals needing ongoing care coordinated through multidisciplinary processes involving multiple systems. This position may also be involved in the Emergency and Involuntary Commitment for Substance Use treatment processes under section 27-81-111, C.R.S. and 27-81-112, C.R.S. respectively.</p> <p>Duties of the position:</p> <p>Care Coordination for Individuals Certified and in Need of Ongoing Care and Priority Populations</p> <p>This position works closely with psychiatric hospitals, the justice system, and the Office of Civil and Forensic Mental Health (OCFMH) when they are working with individuals on emergency mental health holds that may expire prior to finding an appropriate placement for evaluation and treatment. </p> <p>Collaborates with the program manager for transitional support to ensure there is continuity of care and coordination. This position provides care coordination for individuals under 27-65 under involuntary mental health treatment, and priority populations identified by the Behavioral Health Administration. The position requires knowledge of established theories, principles, conceptual models, and professional standards of care coordination. This position collaborates with other staff in BHA Programs to assist with the priority populations that require additional training and support considerations. </p> <p>This position interacts regularly with stakeholders who are interested in services and service outcomes for the population defined as this position's area of expertise. Strong customer service skills are needed to assist all stakeholders. </p> <p>Cross Divisional Coordination</p> <p>This individual may be collecting and organizing metrics from identified sources or they are responsible for the tasks performed by others who have been delegated the duties.</p> <p>General inquiry processes are within the scope of this position and the Director of Care Coordination. Cross-division coordination is necessary based on the entry point of the inquiry, question, concern or complaint, or possible facilitation of care coordination. It may be necessary to collaborate with other functional work units, teams, or divisions to address the stakeholder.</p> <p>Education/Licensure/Certification/Experience:</p> <p>A bachelor's or master's degree in social work, counseling, or a related behavioral health field from an accredited college or university is required.</p> <p>Current, valid Colorado licensure as a:</p> <ul> <li> <p>Licensed Social Worker (LSW) OR</p> </li><li> <p>Licensed Clinical Social Worker (LCSW) OR</p> </li><li> <p>Licensed Professional Counselor (LPC) OR </p> </li><li> <p>Licensed Marriage and Family Therapist (LMFT)</p> </li><li> <p>Licensed Addiction Counselor (LAC)</p> </li></ul> <p>And one (1) year of post-licensure professional experience in behavioral healthcare, specifically in care coordination, case management, or a related clinical capacity. This experience must include working with individuals on emergency mental health holds and certifications under CRS 27-65, as well as an understanding of the Colorado behavioral health system.</p> <p>Required Knowledge:</p> <ul> <li>Clinical Expertise: Comprehensive knowledge of social work/counseling principles, including assessment, diagnosis, treatment planning, and crisis intervention. In-depth knowledge of CO statutes (CRS 27-65, CRS 27-81) and behavioral health regulations. </li><li>Behavioral Health System & Care Coordination: Expertise in care coordination, case management, and transitional support for complex behavioral health cases, with strong knowledge of the CO public behavioral health system (providers, facilities, BHA, and Health Care Policy and Finance). </li></ul> <p>Required Skills:</p> <ul> <li>Communication & Collaboration: Excellent verbal/written communication for internal/external collaboration (EMS, providers, state agencies, clients); ability to tactfully address complex issues and work collaboratively across units, divisions, and with external partners for seamless care. </li><li>Problem-Solving & Organization: Ability to analyze complex situations, identify resources/solutions, apply clinical knowledge (e.g., appropriate placement for individuals in crisis), and strong organizational skills to manage cases, collect metrics, and handle administrative duties. </li></ul> <p>Required Abilities:</p> <ul> <li>Autonomy & Adaptability: Ability to make independent clinical decisions, provide expert assistance without constant supervision, adapt to new assignments/projects, and engage in solo/group work. </li><li>Customer Service: Ability to provide strong customer service to all stakeholders and maintain professional relationships while addressing sensitive issues. </li></ul> <p>Preferred Qualifications:</p> <ul> <li>Experience in interpreting data, contract oversight, and program development. </li><li>Experience contributing to or leading program development, process improvement, or policy review within a behavioral health context. </li><li>At least three years of professional post-licensure experience in care coordination within a public or private behavioral health setting. This includes experience with involuntary mental health certifications (CRS 27-65) and/or substance use commitments (CRS 27-81). In-depth, demonstrated knowledge of the broader Colorado behavioral health system, including specific familiarity with the Behavioral Health Administration (BHA), Behavioral Health Administration Service Organizations (BHASOs), forensic mental health, and the roles of other state agencies. </li></ul> <p>Conditions of Employment:</p> <ul> <li>This position is delegated as a hybrid position.
