Rising Ground is headquartered in New York City.</p> <p>About the Role</p> <p>The PRS Level 2 Case Worker supports UC and their sponsors who present moderate risk factors and need in- depth case management. This position involves conducting both virtual and in-person visits, implementing safety plans, and ensuring follow-up on legal, educational and mental health needs</p> <p>What You'll Do</p> <ul> <li>Conduct comprehensive in person and virtual visits to assess safety and service engagement </li><li>Submit timely, accurate PRS reports and documentation </li><li>Collaborate with community service providers and stakeholders </li><li>Create and monitor safety and supervision plans </li><li>Participate in step-up meetings when higher-level services are needed </li><li>Address moderate challenges such as school enrollment issues, immigration court compliance, or housing instability </li></ul> <p>Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities required of the employee for this job.
