<p>Overview</p> <p>The Workability Care Coordinator works closely with Injured Workers (IW), providers, supervisors, and insurance company adjusters and Case Managers to facilitate timely access to needed care for post-injury recovery, continued coverage of recommended treatment, and facilitation of return to work. They identify opportunities and coordinate clinical care of D-H employees with identified work injuries. They contribute to developing and maintaining the workability program, promote effective resource utilization, and ensure support of the Americans with Disabilities Act (ADA) DH accommodations procedures.</p> <p>This is a 60% Workability and 40% Benefits position, as outlined below.</p> <p>We are seeking candidates who are detail-oriented and compassionate to work on a small team of 3 people. Candidates should have strong written and verbal communication skills, be confident on the phone, and demonstrate empathy and professionalism in presenting and sharing information.</p> <p>This is a full-time, 40-hour/week benefitted position working Monday - Friday from 7:30am - 4:00pm. This position can be located at our Nashua, NH Clinic or Lebanon, NH campus.</p> <p>Responsibilities</p> <p>Workability</p> <p>Follows up on assigned complex employee post-injury cases to assess status and determine the next step. Identifies and facilitates injury prevention by working with the Environmental Health and Safety team and conducting routine OSHA log review and maintenance for recordkeeping. Works with supervisor, benefits, risk management, and other D-HH stakeholders. Maintains D-H and Insurance claim documentation in respective portals and coordinates light duty and alternate work as needed. Manages and coordinates clinical care of high-risk and complex injuries in the outpatient setting. Performs outreach and needs assessments, validates medication, and assesses gaps in care to develop individualized care plans in alignment with patient goals and expectations. Barriers to care are assessed and connections to local assistance to patient with Workers Compensation (WC) related concerns; denials; Return To Work; coordination of care; timely appointments; treatment locations in their area; their responsibilities as injured workers; transparency of Department of Labor (DOL); requesting a hearing etc. Review patient's understanding of injury and plan of care, encourage communication between IW, WC carrier, and the employer regarding injured worker recovery, provide mental health "First Aid" as needed to address, loss of work and work structure, loss of function, financial stress and concerns about return to work, and assist in navigating systems internal and external to DH. Works to assess and improve systems of care that support the care and treatment of Injured Workers through quality improvement processes and informational sessions to groups such as Grand Rounds, Reception and administrative staff to answer questions and to improve communication and management of IW's. Work with IW and their providers, all DH staff and insurance company adjusters and Case Managers to facilitate timely access to needed care for post-injury recovery, continued coverage of recommended treatment and facilitation of return to work. Assesses registry and other reports to extract appropriate high risk and complex cases. Participates in case conferences with care team and the carrier. Understands the DOL and WC guidance and rules, is able to articulate all guidance to injured worker. All other duties as assigned</p> <p>Benefits</p> <p>Support the Americans with Disabilities Act (ADA) DH accommodations procedures Partners with the DH Leaves Administration Team and Benefits Operations Manager Monitors DH Leaves electronic mail for initiated At Work Accommodations and returning employees to work from leaves as an accommodation with restrictions. Collaborates with DH's third-party administrator for necessary information required for completion of eligibility, requests, extensions and return to work. Review and document all ADA with applicable accommodations into DH's leave and disability tracking system utilizing Smartsheet and/or ServiceNow. Ensure adherence to DH policies regarding ADA compliance are followed Collaborate with DH's third-party administrator for necessary information required for completion of eligibility, requests, extensions and return to work. Communicates with employee's supervisor for ability to accommodate and confirm determinations. Advises managers and supervisors on specific employee situations, answering questions pertaining to ADA, as it relates to accommodation needs. Provides risk assessment to managers for denials of accommodations. Partners with DH's third-party administrator for alternatives, as well as legal counsel to help reduce any potential risk related to undue hardships. Maintains proper documentation of correspondence and communications to employee's leave/disability record. Interface with eTime (time and attendance system), PeopleSoft (HRIS employee profiles), Smartsheet, ServiceNow, and DH's third-party administrator of leaves and disability claims management. Conduct daily audits to reconcile data, timely responses for determinations, and system alignment between DH's third-party administrator and DH's tracking system to resolve inconsistencies. Facilitate at work accommodations All other duties as assigned</p> <p>Qualifications</p> <ul> <li>Bachelor's degree or equivalent in education and experience, with 3 years of clinical experience required. </li><li>Worker's Compensation and case management experience desirable. </li><li>Excellent assessment, discrete communication, interpersonal, and organizational/time management skills. </li><li>Demonstrated ability to work well as a member of a multidisciplinary team and respond calmly and effectively in a fast-paced environment. </li><li>Excellent verbal and written communication skills. </li><li>Sound decision making, judgment, time management and negotiating skills. </li><li>Familiarity with electronic medical records, and computer applications including MS Word and Excel. </li><li>Must demonstrate passion for care of employees with work injuries. </li><li>Sound decision making, judgment, and negotiating skills. </li><li>Knowledge of methods to educate and counsel patients, assess their readiness for changing health behaviors. </li></ul> <p>Required Licensure/Certifications</p> <ul> <li>None </li></ul>
