<p>Company:</p> <p>Bon Secours by Compassus</p> <p>Position Summary</p> <p>The Home Health Social Worker provides preventive, educational, evaluative, and treatment services to meet the psychosocial needs of patients and their families, to help them improve, restore, and maintain their maximum level of coping. S/he utilizes community resources and the interdisciplinary team to aid in this process.</p> <p>Position Specific Responsibilities</p> <ul> <li>• Communicates identified needs and potential solutions to clinical team and supervisor. </li><li>Functions as an interdisciplinary team (IDT) member and attends and participates in staff meetings, educational programs, and community events, as requested. </li><li>Completes psychosocial assessment on each assigned patient, as indicated by corporate policy and documents findings in a timely manner </li><li>Completes Support Services Assessment, as appropriate. </li><li>Reviews and explains home health services, as appropriate, to patient's financial classification. </li><li>Develops and updates care plans. </li><li>Visits patients, as indicated. </li><li>Monitors hospitalizations and Medicare Part A admissions. </li><li>Provides direct social work services. </li><li>Obtains physicians order for Home Health Social Services. </li><li>Facilitates placements and monitors patient's adjustment. </li><li>Coordinates care giving issues. </li><li>Facilitates discharge planning. </li><li>Explains and assists with advance directives. </li><li>Addresses financial concerns and completed financial assessment. </li><li>Contacts community agencies, as appropriate. </li><li>Initiates Medicaid spend down. </li><li>Assesses appropriateness for Special Concern supplies. </li><li>Evaluates insurance concerns. </li><li>Develops and utilizes community resources. </li><li>Supervises social work practicum students. </li><li>Develops and maintains working relationship with community agencies, i.e., long term care facilities, DFS, hospitals, social security office, V.A., etc. </li><li>Utilizes available community resources to meet family needs. </li><li>Works in cooperation with community agencies and lay groups. </li><li>Provides education and in-services to contract agencies, as appropriate. </li><li>Provides counseling to patient and family. </li><li>Provides counseling to meet psychosocial needs of patient and family. </li><li>Collaborates with and provides information to home health interdisciplinary team related to counseling issues. </li><li>Provides crisis intervention to patient/family. </li><li>Refers to community counseling agencies, as indicated. </li><li>Offers and facilitates memorial services, as directed. </li><li>Performs other duties as assigned. </li><li></li></ul> <p>Education and/or Experience</p> <ul> <li>Bachelor of Science in Social Work required, or Master of Science in Social Work degree preferred; unless otherwise stated by state-specific requirements. </li><li>Bachelor's degree in Psychology, Sociology, or other field related to social work in addition to one (1) year of social work experience in a healthcare setting and is supervised by an MSW may be considered; unless otherwise stated by state specific requirements. </li><li>At least one (1) year of social work experience in a healthcare setting required. </li></ul> <p>At Compassus, including all Compassus affiliates, diversity, equity, and inclusion are fundamental to our Pillars of Success.
