<p>Summary:</p> <p>The Memory Care Coordinator (MCC) leads, coordinates and manages resident's life in the Memory Care neighborhood through a comprehensive, person-centered approach. The MCC will work with the nursing staff to provide continuity of care and services within the Memory Care neighborhood and as defined in policies, guidelines and regulatory bodies. The MCC reports directly to the Memory Care Manager or Executive Director.</p> <p>Essential Responsibilities:</p> <ul> <li>Incorporate Capri Blu Grotto policies, program and systems within the neighborhood </li><li>Work with the Blu Grotto care staff for Blu Grotto policies and system compliance </li><li>Work with managers to ensure programs are provided to create a positive and memorable resident experience </li><li>Direct issues and concerns to the Memory Care Manager, nurse or ED </li><li>Support team members to inspire compassionate performance and positive relationships with residents and families </li><li>Model resident engagement and proper technique of approach when in the neighborhoods </li><li>Provide guidance and support to caregivers regarding program delivery as they are assigned </li><li>Assist to coordinate and deliver care team continuing education regarding Dementia Support </li><li>Work closely with the Memory Care Manager and participate in scheduled Blu Grotto meetings when assigned </li></ul> <p>Resident Care:</p> <ul> <li>Connect with residents in a genuine, compassionate and memorable way </li><li>Observe and note resident's ability and make program recommendations which support the resident's highest level of function </li><li>Discuss pertinent assessment information, changes or additions with the MCM or nurse so he/she can make the necessary changes in the ISP and/or assessment </li><li>Obtain new resident's life story then develop and implement the resident's move-in plan </li><li>Work with the resident, MCM or nurse, family and team members to develop and implement designed behavior/emotional well-being service plan strategies </li><li>Attend Blu Grotto service plan meetings for neighborhood residents </li><li>Be attentive to resident's overall well-being and make recommendations for adjustments </li></ul> <p>Programs:</p> <ul> <li>Work with management to purchase supplies and equipment </li><li>Develop Blu Grotto program calendar for 7 days a week delivery including evenings </li><li>Work closely with the Memory Care Manager or Lifestyle Enrichment designee to coordinate special events, entertainers and outings </li><li>Coordinate meaningful, ability based, equal opportunity outings for the residents of Blu Grotto </li></ul> <p>Skills Abilities, Qualities:</p> <ul> <li>Passion for working with individuals with dementia </li><li>Positive and motivating </li><li>Ability to deal tactfully and cheerfully with care staff, residents and family members </li><li>Possesses good interpersonal organizational and communication skills </li><li>Must possess the ability to plan, develop, organize and implement programs </li><li>Ability to handle several tasks and respond effectively to unexpected changes </li></ul> <p>Education and/or Experience</p> <ul> <li>Completion of or willingness to complete memory care courses as well as on-going education </li><li>Preferred minimum of 1-year experience in delivering leisure and recreational programs </li><li>Preferred minimum of 2-years' experience working with individuals with a diagnosis of dementia </li></ul>
