<p>Job Responsibilities</p> <p>This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Region C (Pee Dee and Waccamaw Mental Health Centers).</p> <p>The Office of Mental Health's mission is to support the recovery of people with mental illnesses. We provide a complete array of medical and support services for children, adults, and families throughout South Carolina.</p> <p>As the Client Advocate II, working with under general supervision of the Director of the Office of Care Coordination, you will effectively coordinate the overall care of clients with mental health conditions, ensuring appropriate and cost-effective services. Clients may involve individuals with mental illness, adults, and children, to include those with physical, vocational, educational, social, and other needs in addition to mental health needs. You will primarily be serving the Highway to Hope patients.</p> <p>Responsibilities for the Client Advocate II:</p> <ul> <li>Assist referred clients to access needed services as identified by needs assessment. </li><li>Develop a comprehensive Case Management Plan (CMP) identifying specific goals and actions corresponding to needs identified in the Comprehensive Needs Assessment (CNA). </li><li>Follow up and monitor to ensure CMP is effective. </li><li>Consistently adheres to Office of Clinical Care Coordination productivity standards. </li><li>Properly renders, documents, and bill all services allowable per federal, state, CMS, HHS, third party payor regulations, and DMH Corporate Compliance policies and procedures. </li><li>Meet all Pathlore and Center/DMH training requirements, including annual security training. Demonstrate cultural awareness when serving clients, interacting with peers, or meeting with community partners. </li></ul> <p>Minimum and Additional Requirements</p> <ul> <li>A bachelor's degree and professional experience in human services or social services programs. </li><li>A degree in a health or human services field that promotes the physical, psychosocial and/or vocational well being of the individual being served. </li><li>Documentation of at least one (1) year of experience working with the target population. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Bilingual abilities in English and Spanish (or another language) are a plus. </li></ul> <p>Additional Comments</p> <p>Post hire, employee must:</p> <ul> <li>Have the ability to access multi-disciplinary staff when needed. </li><li>Have documented experience, skills or training in crisis intervention, effective communication and cultural diversity and competency. </li><li>Possess knowledge of community resources and a working knowledge of families and/or systems theory. </li><li>Travel within and outside the CMHC catchment and where there is need for services. </li></ul> <p>The Department of Behavioral Health and Developmental Disabilities is committed to providing equal employment opportunities to all applicants and does not discriminate on the basis of race, color, religion, sex (including pregnancy, childbirth, or related medical conditions including, but not limited, to lactation), national origin, age (40 or older), disability or genetic information.</p> <p>Supplemental questions are considered part of your official application.
