Be the difference.</p> <p>Job Summary</p> <p>Provides direct and indirect counseling services to patients and their families using individual, group, and family therapy. Provides bio-psychosocial assessments, risk assessments, treatment planning and coordination with multidisciplinary team. Be the difference</p> </li><li> <p>Provides direct and indirect clinical/therapy services to patients and families covering a broad range of standard practice modalities and theories, biopsychosocial assessments, diagnostic determinations, individual, group, and family therapy, aftercare services.</p> </li><li> <p>Participates in the planning, development, and implementation of the therapeutic program, including strategies for quality improvement, patient satisfaction, regulatory adherence, and associated policies, processes, chart audits.</p> </li><li> <p>Develops an individualized treatment plan including measurable treatment goals related to patient's presenting problems. Participates as an active member of the multi-disciplinary treatment team.</p> </li><li> <p>Performs case management responsibilities involved with patient care. This includes, but is not limited to, Utilization Review, Discharge Planning, Family Involvement/Education, timely and appropriate documentation as needed.</p> </li><li> <p>Serves as team lead and operates as a resource to staff. Supervises graduate students as field instructor/site supervisor.</p> </li><li> <p>Performs other duties as assigned.</p> </li></ul> <p>Supervisory/Management Responsibility</p> <ul> <li>This is a non-management job that will report to a supervisor, manager, director, or executive. </li></ul> <p>Minimum Requirements</p> <ul> <li> <p>Education - Master's degree in Social Work, Counseling, Psychology or related human services field of study.</p> </li><li> <p>Experience - Two (2) years of experience providing direct counseling care in a behavioral care setting. Hospital or community agency setting preferred.</p> </li></ul> <p>In Lieu Of</p> <ul> <li>Team members employed in this job prior to July 1, 2020, are grandfathered under prior educational and experience requirements.
