If so, we invite you to join our Lakeland team at Compassionate Care Hospice, an Amedisys Company, one of the largest and most trusted home health and hospice companies in the U.S. As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families. Our hospice care focuses on the whole person - combining medical care and support for emotional and spiritual needs, as well as support for loved ones and caregivers. Our nurses say they are following their calling and have a "Hospice Heart." Attractive Compensation Package $80,000 - $85,000 plus add on bonus May vary depending on location, job-related knowledge, skills, and experience. Orientation, Onboarding & Training You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an R.N is 7-9 weeks, however, is individualized according to your needs and experience. Technology Use Hospice RN Case Managers utilize and leverage: HCHB = Proprietary Real-time database Optum Pharmacy solutions with 24/7 access IPrescribe - efficiencies with medication management Forcura - Document management system Medline - supply ordering system TalentEDGE - Amedisys' premier learning center EBSCO - Nursing Resource Center for CEUs Tablet with voice dictation and mileage tracking Outlook added to Teams, Chat, Yammer Responsibilities Performs patient assessments and collaborates with the care team to develop and implement a plan of care. Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Teaches patients, families, and caregivers appropriate patient care techniques. Regularly communicates with clinical manager and care team. Performs on-call responsibilities and on-call services to patients/families as assigned. Other duties as assigned. Qualifications One year of nursing experience in acute care settings. Current RN license, specific to the state(s) you are assigned to work. Current CPR certification. Valid driver's license, reliable transportation and liability insurance.
