For some, that place is home-and we're more than happy to meet you there.</p> <p>To learn more: https://wcathome.com/</p> <p>The Home Health Licensed Practical Nurse (LPN) provides skilled nursing care with the goal of restoring, maintaining and promoting optimal mental and physical health of patients through assessment, teaching, counseling and appropriate preventative and rehabilitative nursing measures.</p> <p>What You'll Do In This Role:</p> <ul> <li>Conduct on-going assessments of the patient's physical, psychological and social needs. </li><li>Include the patient's family and/or significant other, when indicated, in the assessment of the patient's needs. </li><li>Ensure documentation meets reimbursement criteria and adhere to agency policies and guidelines. </li><li>Communicate changes in condition/needs in a timely manner to appropriate agency personnel, physicians, patient's family/caregiver, and outside resources as indicated. </li><li>Communicate and collaborate with members of the health care team through case conferences to meet patient needs, when indicated. </li><li>Demonstrate knowledge of community resources and utilize them appropriately. </li><li>Plan individualized patient care utilizing professional knowledge. </li><li>Organize and analyze data collected to identify patient needs. </li><li>All other duties as assigned. </li></ul> <p>Core Competencies</p> <ul> <li> <p>Demonstrate sound judgment and effective decision-making when prioritizing responsibilities.</p> </li><li> <p>Take initiative and proactively support peers and leadership as needed.</p> </li><li> <p>Pursue ongoing learning opportunities to enhance nursing skills and clinical expertise.</p> </li></ul> <p>Physical Requirements:</p> <ul> <li>Ability to perform cardiopulmonary resuscitation and relief of an obstructed airway. </li><li>Ability to climb stairs and to stand and/or walk for a period of time. </li></ul> <p>The Experience You'll Bring</p> <p>Required:</p> <ul> <li>Active Virginia (or compact) unencumbered LPN License. </li><li>At least one-year experience in acute clinical care. </li><li>Ability to meet deadlines while working under limited supervision. </li><li>Current CPR certification. </li><li>Valid state driver's license. </li><li>Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers. </li><li>Proof of motor vehicle insurance. </li><li>Flu-vaccinated </li></ul> <p>Preferred:</p> <ul> <li>Recent experience in home health. </li><li>COVID-19 Vaccinated. As of now, the 2025-2026 COVID-19 vaccine formulation is being distributed to providers and pharmacies; however, it has not yet received official approval from the CDC. In Virginia, pharmacists may administer COVID-19 vaccines under the Virginia Board of Pharmacy's statewide protocols or with a direct order from a licensed medical provider. We encourage all new hires to consult with their healthcare provider regarding the most current vaccination recommendations.
