<p>Admissions Coordinator - Skilled Nursing Facility (SNF)Ravenwood Specialty Care - Waterloo, IA About UsCare Initiatives is Iowa's largest non-profit senior care provider, operating over 40 skilled nursing facilities (SNFs), rehabilitation centers, and long-term care communities statewide. We are committed to providing high-quality care, rehabilitation services, and support for seniors and their families at every stage of the healthcare journey. Position SummaryThe Admissions Coordinator is responsible for managing the resident admission process for our skilled nursing facility. This role works closely with hospitals, rehabilitation centers, physicians, case managers, and referral sources to ensure smooth transitions for residents. The position requires strong relationship-building, marketing, and customer service skills, along with a thorough understanding of long-term care regulations and admission procedures. Key Responsibilities</p> <p>Coordinate and oversee the admissions process from initial inquiry to move-in. Screen and review healthcare referrals to determine clinical and financial eligibility. Communicate with residents, families, hospital discharge planners, and healthcare providers to ensure a smooth transition into care. Maintain compliance with state and federal regulations for SNF admissions. Ensure all admission documentation is accurate, complete, and in accordance with CMS guidelines. Maintain confidentiality of resident information in compliance with HIPAA.</p> <p>Marketing & Community Outreach</p> <p>Implement marketing and census development strategies to increase occupancy. Develop and maintain relationships with hospitals, rehabilitation facilities, physicians, social workers, and case managers. Attend and represent the facility at healthcare networking events, community outreach programs, and professional associations. Deliver presentations to community organizations about skilled nursing, rehabilitation, long-term care, and aging services. Monitor competitor activity and provide market insight to facility leadership.</p> <p>Qualifications</p> <p>Nursing license (RN or LPN) highly preferred. Experience in skilled nursing, long-term care, admissions, healthcare marketing, or social services is strongly desired. Knowledge of Medicare, Medicaid, managed care, and private insurance requirements for admission. Excellent communication, customer service, and relationship management skills.
