<p>Additional Position Details: FTE: 1.000000 | Full Time | This position is primarily located in Grants Pass, with most of the RNs time spent at ATRMC. Hours for this position are 8:00am-4:30pm Monday, Tuesday, Thursday, Friday and Saturday.</p> <p>The Asante Hospice Mission is to serve our community by providing quality end-of-life care. We honor our patients' and families' choices through compassionate holistic services.</p> <p>Our Hospice nurses are independent, self - motivated, committed to ongoing personal and professional growth, have excellent communication skills, and have a passion for helping people and making a real difference in their daily lives.</p> <p>Asante Hospice has one of the highest employee engagement and retention rates within the Asante organization, which reflects the support of the Leadership, the hospice team, and the challenging and rewarding work of end-of-life care.</p> <p>Hospice Liaison RN - Position Summary</p> <p>The Asante Hospice Liaison RN, evaluates and facilitates Hospice referrals from the systems hospitals, APP clinics and community referrals. This RN assesses patients for hospice eligibility and coordinates the transfer of inpatient information to the hospice office; collects and processes information for Hospice referrals; and alerts appropriate team members for necessary coordination of care.</p> <p>As a Hospice Liaison RN for Asante - you will have the opportunity to:</p> <ul> <li>Collaborate with referral sources, resource management team members, Hospice Medical Director as well as discharging hospitalists, DME provider and/or scheduler to facilitate transition of care. </li><li>Assess patient and families physical, psychosocial and emotional condition to determine patients' safety and the care needs in the home setting or setting in which the patient will reside. </li><li>Promote a positive working relationship by collaborating with peers throughout the system by maintaining open lines of communication and accepting and providing constructive feedback in a supportive and nonjudgmental manner. </li><li>Provide ongoing assessment of patients receiving hospice general inpatient level of care or a non-related condition and provide coordination in transfer of patients to GIP status. </li><li>Contact patient/family to explain the hospice benefits/services and identify if the patient/family agree with hospice and palliative care vs curative or aggressive treatment. </li><li>Discuss goals of care conversations with prospective hospice patients and their family members. </li><li>Develop and maintain relationships with resource management team members, community referral sources such as ALF, RCF, NF and Memory care facilities throughout the communities in which Asante Hospice serves and facilitate and provide education to the care facilities and provide support for team members as needed utilizing the Hospice Team members as indicated. </li></ul> <p>Qualifications</p> <p>Education</p> <ul> <li>Bachelor's in nursing (BSN) preferred </li></ul> <p>Experience</p> <ul> <li>2 years clinical hospital experience as an RN and 1 year current Hospice, Palliative care, or Home Care experience; required </li></ul> <p>License/Certification</p> <ul> <li>Registered Nurse (RN) licensed by the Oregon State Board of Nursing required upon start </li><li>Basic Life Support for the Healthcare Provider (BLS) certified through the American Heart Association (AHA) required upon start </li><li>Possession of a valid Driver's License with proof of safe driving record for the past ten years and proof of insurance required </li></ul> <p>What We Offer for Full-time and Part-time Position</p> <ul> <li>Earn a competitive and progressive salary </li><li>Benefit from health plans that are focused on health and wellness including medical, dental, vision and wellness beginning within 30 days of hire </li><li>On-call employees may receive an additional 15% for flexibility </li><li>Plan for your future with a retirement package that includes up to 6% employer contribution </li><li>Experience a healthy work-life balance with our generous earned time off (ETO) </li><li>Continue to enhance your education through our tuition reimbursement and tuition repayment plans </li></ul> <p>Why Asante</p> <p>Asante is more than a great hospital system in an amazing location.
